Are you an experienced clinician looking to utilise your skills and experience in a fast-paced environment? Are you able to handle multiple sources of information simultaneously and communicate the outcome of your work clearly across multiple teams and organisations? This job opportunity will see you work closely with multi-organisation partners to improve the patient experience and timeliness to the most appropriate health and care services.

Main duties of the job

We are looking to recruit an experienced clinician into our friendly team as the service continues to grow and develop. If you have experience of working in acute, community or rehab services dealing with complex challenges and are keen to do something different in a highly performing, stimulating and fast-paced environment then this may be your opportunity.

You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner.

Qualifications

Essential criteria
  • Registered Nurse with current UK NMC registration / Current AHP with UK HCPC registration
  • Additional professional qualifications in relevant fields e.g. Continuing Health Care / community rehabilitation discharge planning
  • Demonstrable successful innovation in clinical practice
Desirable criteria
  • Evidence of service improvement within previous role

Experience

Essential criteria
  • Previous experience working within either an acute hospital or rehab unit or community service
  • Able to communicate with patients, relatives and MDT and on occasions deal with conflicting views
Desirable criteria
  • • Knowledge of common causes of delays which lead to a delayed transfer of care (DTOC) for patients
