Job overview

Are you a Community Psychiatric Nurse/Mental Health Practitioner or newly qualified with a keen interest in developing your skills? We’re looking for an individual to work in our Older Adults Service so you will need to be passionate about caring for patients with mental health needs including dementia.  

You will work in our Older Adults Home Treatment Team providing mental health care and treatment using appropriate pathways including the Dementia Living well Strategy in line with NICE guidelines.  

You will have excellent people skills and a good understanding of Mental Health Framework and legislation in addition to Clinical Risk policies and procedures.  

This role is full-time (37.5 hours per week), however interested part-time applicants will be considered and you will be supported by a dedicated team that strives to create a compassionate environment. 

This service operates 7 days per week between the hours of 9am and 7pm, so flexibility with regard to shift patterns, weekends and bank holidays will be required.  

Main duties of the job

  • Participate as a member of a multi-disciplinary team managing a caseload of patients  
  • Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.  
  • To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.  
  • Direct and supervise Support Workers and/or students.  
  • Provide advice to other members of the service and agencies on the needs of service users.
