Community Case Manager (Urgent Community Response)

An exciting opportunity has arisen within the Urgent Community Response Directorate to recruit two Community Case Managers within the Urgent Community Response Services (UCRS) Team.

We are seeking to appoint a dynamic, driven, motivated and caring individual who will bring their experience and personability to the role to ensure that as a team we align to the Trusts People, Patient, Population Strategy.

Our current UCRS Team consists of a range of multidisciplinary colleagues including:

·       ACPs

·       Consultants

·       Occupational Therapists

·       Physiotherapists

·       Operational Managers

Within the UCRS Team, we deliver a range of services including:

·       Falls Service/Falls Pick Up/Wound Closure

·       Admission Avoidance (2-48 Hour Response)

·       Community DVT Pathway

·       2 Hour Urgent Response

·       8x8 Navigation Centre

The posts will support people in their own homes who are experiencing an exacerbation in their health. You will, therefore, have a passion for community nursing and urgent care pathways.

Our new Midland Metropolitan University Hospital opens October 2024, and will not only provide excellent patient care from a modern acute setting but will look to support the wider communities by supporting with education access with the development of the Learning Academy which is set to be opened at MMUH.

Please be aware that our base location may change following the opening of MMUH, but this will be discussed further at Interview.

Reporting to a senior Community Matron for the service, the post holder will be an autonomous practitioner and clinical specialist responsible for developing and implementing service provision within the  team across the borough of Sandwell.

The post holder will have continuing responsibility for proactively monitoring and managing patients on a caseload within a designated pathway, recognising the early symptoms of disease exacerbation, acute illness, and injuries, as well as social and familial factors based on the understanding of long-term conditions, the effects on the individual, their family and environment as well as current evidence and practice standards.

The service will be delivered to a defined population of patients needing complex case management for long term conditions and complex social risk factors within the community setting, preventing inappropriate hospital admission.

The post holder will also be ultimately responsible for the management/decision making about care and service provision, for individual patient pathways across the primary and secondary care interface.

  • Qualified Nurse Prescriber or willing to study for
  • Evidence of Continued Professional Development
  • Hold a recognised teaching/assessing qualification.
  • Relevant qualification/ module at first degree level

Experience

  • Specialist expertise / knowledge with underpinning theory of long-term conditions

  • Experience of working as part of a multi-disciplinary team
  • Experience of contributing to the personal development of less experienced staff, students or assistants
  • Ability to work across professional and organisational boundaries.

  • Able to carry out frequent, long periods of medium-intense concentration carrying out assessments, planning treatment programmes, supervising support workers and more junior staff and dealing with complex equipment
  • Able to deal sensitively with patients with high levels of anxiety/aggression which may be due to dementia, sedation, limited mobility or who are unable to accept current clinical outcomes.
