Job overview

NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly too.

The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.

NHS@Home has six pathways, covering a wide range of conditions including frailty, respiratory, heart failure, and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.

The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

Working in our team you will learn a very wide set of acute care skills in a community setting. You’ll also contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people.

 

Main duties of the job

Registered Nurse – Band 5

 Working within the NHS@Home service, your duties will include:

  • Poviding acute care to patients with complex and specialist needs in the community, across Bristol, South Gloucestershire and North Somerset.
  • Working alongside the multidisciplinary team to develop, implement and evaluate care plans.
  • Assessing the health and psychosocial needs of the patient within the hospital and community setting.
  • Plan and implement treatment and use a wide range of service to meet their needs.
  • Utilising a wide range of clinical skills to support the ongoing management of patients in their home environment e.g. delivering IV therapies, venepuncture, cannulation and wound care (not exhaustive list).
  • Communicating effectively, networking and liaising to ensure high quality responsive, safe, efficient and compassionate care is provided for our patients, carers and families.

The post holder will work in collaboration with the NHS@Home team at Southmead hospital and BNSSG system partners at Sirona and UHBW. Hours of operation are 0700-21:30

It is essential you hold a valid full UK driving license and have access to an insured car with business insurance. Travel expenses will be paid.

Qualifications

Essential criteria
  • RGN with current UK recognised, live Registration with NMC PIN
  • DipHe Adult Nursing equivalent or above
Desirable criteria
  • Teaching and Assessing qualification

Work experience

Essential criteria
  • Is competent in a range of clinical skills including venepuncuture, cannulation, VAC therapy, CVC device management
Desirable criteria
  • Vared registered clinical back ground with over 12 months post qualification experience
  • Currently working within or community setting as a qualified nurse
  • Experience of working in multidisciplinary teams

Kowledge/skills/ abilities

Essential criteria
  • Advanced communication skills, verbal and written including record keeping
  • Excellent interpersonal skills
  • Evidence of continuing professional development
  • Able to undertake shift work including weekends
  • Sound knowledge of the guidelines set out by the NMC code of professional conduct and current nursing issues.
  • Sound knowledge of Trust policies & practices including mandatory requirements.
Desirable criteria
  • Basic IT and computer skills, expected to develop these in line with service needs.

Personal qualities/ special circumstances

Essential criteria
  • Has a full UK driving license and ass access to an insured car daily.
  • Able to work within a diverse community of patients and health care professionals.
  • Able to work and remain calm under pressure.
  • Punctual/ flexible
