Job overview

An exciting opportunity has arisen for a Health & Wellbeing Facilitator to work as part of the new Hywel Dda University Health Board Pre-Diabetes Service. These roles will be located primarily within Carmarthen/South Pembrokeshire and Aberystwyth with an expectation to be able to cover Health Board wide where required.

The Health & Wellbeing Facilitator role is critical to supporting the implementation, delivery and evaluation of the Diabetes prevention programme.

The post holder will work independently with people at the risk of developing Type 2 Diabetes. This work will include the  delivery of 1-1 brief intervention, delivering the Foodwise for Life Programme, supporting lifestyle changes through exercise and lifestyle changes specific to pre-diabetes which are meaningful to the individual and may include for example, skills development techniques, providing support and advice.
Part of a Multi-disciplinary team with health professionals across primary, community and secondary care. Local Authority, National Exercise Referral Scheme, Public Health Wales (Local Public Health Team), and the Voluntary Sector.

The successful candidate will be expected to travel and work across a variety of settings to include community & leisure facilities.

Main duties of the job

As part of the role, the post holder will undertake Brief Intervention to service users identified as at risk of pre-diabetes. The brief intervention will involve empowering individuals to consider changing their health for the better, address motivation, change behaviour and ultimately reduce the health risk of pre-diabetes. 

The post holder will also deliver the ‘foodwise for life’ lifestyle programme, to provide dietary and lifestyle education for clients who ‘opt in’ for additional support following the brief intervention advice and encouragement for a further 8 weeks. ‘Foodwise for Life’ is an eight week structured group programme which utilises evidence based approaches to weight management. 

The post holder will work with people living with long-term /chronic conditions, carers, health professionals across primary, community and secondary care. Also with the Local Authority, National Exercise Referral Scheme, Public Health Wales (Local Public Health Team), and the Voluntary Sector. 

The post will be driven by business and service user need and the post-holder will be required to work flexibly over evenings and Saturdays, and to be flexible  to cover the work of other Health and Wellbeing Facilitators when necessary to ensure no disruption to service delivery.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Qualifications and Knowledge

Essential criteria
  • NVQ Level 4 Diploma or equivalent in Healthcare, or equivalent level of skills and knowledge gained through experience working in health, care, support, educational environments
  • Good awareness of equality and diversity issues
  • Knowledge of the principles and practices involved in behaviour change
  • Demonstrate sound knowledge of the importance of good nutrition health
  • Understanding of the evidence base underpinning routine lifestyle, health improvement practice
  • Further evidence of continual education
  • A commitment to undertaking continuous professional development including training as a Foodwise for Life Facilitator
Desirable criteria
  • Motivational interviewing
  • Agored Cymru, level 2 Community Food and Nutrition Skills
  • Foodwise facilitation accreditation
  • Group educational skills experience
  • Accredited courses in behaviour change
  • Good knowledge of government initiatives about long term conditions and self-care
  • Knowledge of NHS in particular primary care
  • Knowledge of voluntary organisations

Experience

Essential criteria
  • Experience of working in a healthcare and, or community setting
  • Maintaining routine administration systems
  • Experience of communicating effectively with a range of people at different levels
  • Experience of involvement in development work e.g. gathering data for monitoring and evaluating to inform service improvement
Desirable criteria
  • NHS experience
  • Evidence of involvement in supporting nutrition and lifestyle improvement programmes
  • Experience of project work

  • Group educational delivery skills
