Job overview

East Cheshire Specialist Palliative Care Service are looking to recruit an experienced enthusiastic highly skilled Band 7 Macmillan Specialist Palliative Care Nurse who is a dynamic and passionate to work within our integrated team offering specialist palliative care to patients in their own homes and hospital setting in the East Cheshire locality, based in Macclesfield District General Hospital.

As a Band 7 you will undertake proactive caseload reviews to ensure the delivery of high -quality patient care. Excellent leadership and communication skills are also required. Teaching experience is essential to share knowledge, skills and enhance high standards of specialist palliative care, contributing to the service improvement and the wider ambitions of palliative and end-of-life care.

The post holder will link closely with the primary care network, social services and the services delivered within the Acute and Integrated Community Care Directorate.

Candidates are required to have a relevant degree and a recognised post registration qualification in palliative care underpinned by advanced clinical practice.

Candidates must possess V300 or V150 nurse medical prescribing, or already working towards.

Hours per week: 22.5 hours

Main duties of the job

An expert practitioner who can lead the specialist nursing/practitioner function in the delivery of specialist care including assessment of complex care needs and is able to lead in developing and implementing programmes of care. The postholder will take a lead role in facilitating, enabling and supporting the development of this service based on best practice ensuring delivery of high quality, effective and efficient care within the specialist palliative care team. The postholder will lead in developing and delivering high quality patient centered care and working with the associated clinical team to ensure this is incorporated within the directorate and Trust objectives.

The Macmillan specialist palliative care team provide a nursing service in both the hospital and community for those patients with specialist palliative care needs.  The postholder will support patients and provide a service to facilitate and maintain an optimum quality of life for both patients and their relatives.

Qualifications

Essential criteria
  • RGN / RN / RSCN
  • Nursing degree or be working towards
  • Master’s level short courses in specialist area or evidence of working towards or can demonstrate an equivalent level of work experience
  • Teaching / Mentoring qualification
  • Evidence of continuing professional development
  • V300 or V150 nurse prescriber or working towards

Knowledge & Skills

Essential criteria
  • In-depth knowledge of Recent NHS legislation, recommendations for specialist palliative care
  • Detailed awareness of the national and local priorities and changing trends in specialist area
  • Audit skills

Experience

Essential criteria
  • Proven post registration experience, in the relevant specialism
  • Experience of planning and completing audit / research
  • Experience of teaching and mentoring of staff
  • Experience of undertaking projects
  • Experience of implementing change
