Job overview

An exciting opportunity has arisen for a Therapy or Nursing Associate Practitioner  within the East Cheshire Transfer of Care hub  to support timely and effective pathway 1 and 2  discharges. The role will involve working collaboratively with care providers. care homes beds, inpatient therapy teams, social services and integrated discharge team nurses. 

We are looking for someone who has a good eye for detail and a passion for supporting the best outcomes for patients. 

Hour per week 37.50

Main duties of the job

The Therapy/Nursing Associate Practitioner will work independently to implement pathway 1 care packages and  assessments for pathway 2 placements under the supervision of a registered Therapist or Nurse to support the delivery of patient centered approaches and provide compassionate, safe and effective care to a designated caseload /cohort of patients.

The Therapy/Nursing Associate Practitioner will provide holistic and person-centered care and support for people across the sphere of nursing and always communicating effectively within the team, with other health care professionals and with the patient family, carers/ visitors.

The Therapy/Nursing Associate Practitioner will work independently but with the support and leadership of registered nurses and therapists, working within aspects of the nursing/therapy process to ensure high standards and a safe environment are maintained.

Qualifications

Essential criteria
  • Therapy / Nursing Associate Practitioner qualification
  • Evidence of recent professional development in an up-to-date portfolio

Knowledge

Essential criteria
  • Understanding of the scope of the role of the Nursing/Therapy Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Ability to assess clinical situations and act appropriately.
  • Able to assess, plan and evaluate patient care, taking into account the needs and desires of the patients.
  • Awareness of effects of ill health and the ageing process.
Desirable criteria
  • Long term condition management
  • Understanding of discharge to assess pathways
  • Knowledge and experience relevant to area of practice.

Skills & Experience

Essential criteria
  • Recent previous experience within a comparable role/transferable skills
  • Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice.
Desirable criteria
  • Experience of using electronic patient/service user record systems.
  • Experience of working within a multi-disciplinary team.
