Fixed Term from 8th July for 9 months & Secondment for 10 months for internal staff. 

An exciting opportunity has arisen for a Band 5 Staff Nurse to join our Transfer of Care Hub. We are looking for an enthusiastic and experienced Nurse who has excellent communication and assessment skills, is passionate about patient flow and able to work at a fast pace in both a changing and challenging environment.

The successful candidate will be responsible for completing assessments for patients to facilitate discharge from hospital through a Discharge to Assess model. We are looking for individuals who wish to achieve best patient outcomes on discharge from hospital embracing the Home First approach. 

Hours per week: 30 hours for Fixed Term and 22.5 hours for Secondment

Main duties of the job

The post holder will facilitate initial proactive assessments for inpatients in order to facilitate a safe, effective discharge from hospital and prevent avoidable readmission to hospital.

Key aims are

  • Delivering a consistent, coordinated approach to assist patients to identify potential needs at the point of discharge to avoid unnecessary delays in transfer of care
  • Working collaboratively with external and internal partners for the benefit of the patients to ensure smooth transition from hospital and reduce risk of readmission
  • Transparently providing information and expert advice to patients, carers and other professionals
  • Reporting all delayed transfers of care, for criteria to reside reporting .

Qualifications

Essential criteria
  • Current first level registration in General / Adult nursing or Mental Health nursing
  • Experience at band 5
Desirable criteria
  • Experience in an acute hospital environment

Experience

Essential criteria
  • Experience of working collaboratively with other agencies eg Social Services
  • Experience of working within a multi-disciplinary setting
  • Experience of supervision, appraisal and performance review
Desirable criteria
  • Intermediate care experience
  • Experience of case management
  • Experience of inter-disciplinary working
  • Rehabilitation experience
  • Experience of Community Nursing

Knowledge & Skills

Essential criteria
  • Knowledge of National Service Framework for Older People
