Job overview

Dorset Healthcare are looking for Community Rehabilitation Assistant to join our supportive Purbeck Integrated Community Rehabilitation Team based Purbeck.

 The multi-disciplinary team, work with adult patients in their own homes, Community Hospital, or care homes to promote independence and improve quality of life by supporting them to remain at home or be discharged home. Therefore, being a car driver is essential for this role.

As part of this role, you will be required to work as member of the therapy team at Swanage Community Hospital. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence. We offer opportunities for further learning and development, including the chance to apply for Nursing, Physiotherapy and Occupational Therapy Apprenticeships.

Responsibilities will require you to work mainly under the close supervision of the clinicians within the team, but at times also to work autonomously. Previous rehabilitation/care experience, working in the community or on a ward would be desirable.

We have two post available for the right candidates, 1 x full time and 1 x 22.5 hours per week. The hours covered by the service are 08:00-20:00, 7 days per week with weekend enhanced pay!

Main duties of the job

Implement planned Intermediate Care/Rehab for individuals in their own home, Community Hospital ward and other community settings maintaining a high standard of evidence based practice to meet identified needs.

Actively participate in learning opportunities to maintain knowledge and inform practice. Actively seeks feedback.

Manage own workload and time, for example maintaining own diary.

To assist clinicians in administrative tasks required for the efficient day-to-day running of the team.

Record own learning and personal development. Identifies and articulates areas for self-improvement, including through self-reflection and evaluation of practice.

To attend mandatory and statutory training as identified at appraisal and by trust policy.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Essential criteria
  • NVQ / Diploma Level 2/3 in Healthcare or willing to work towards
  • Skills to manage challenging situations
  • Good organisational skills/decision making

Job specific experience

Essential criteria
  • Experience working in Health or social care setting
  • Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
  • Ability to prioritise and organise workload effectively
Desirable criteria
  • Community experience
  • Previous experience with the rehabilitation of Adult/Elderly patients

  • Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
