Are you a registered Nurse, Social Worker or Occupational Therapist looking for a rewarding role where you are truly valued?

If so we have a post available for permanent full time Band 5 Mental Health Practitioner at Alexander House, working in our Community Mental Health Team. 

You will be an active member of our multidisciplinary team (MDT), working as a Key Worker, with a caseload which will be challenging at times, as well as rewarding.  We will offer you the opportunity to work with a highly skilled and experienced MDT providing support, development opportunities and supervision for you, as well as excellent standards of care for our service users and carers.

We are an established team covering Harrogate, Knaresborough, Ripon, Wetherby, working in peoples own homes and other care environments. 

Please note that the standard working hours for this role are Monday to Friday 8.30am to 4.30pm or 9am to 5pm, however flexible working can be considered.

Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.

Main duties of the job

As a Community Practitioner you will have a genuine interest in collaborative working with older adults experiencing functional and/or organic mental health problems including their carers and families.

As a registered professional this is an ideal opportunity for someone who wants to develop their community clinical skills.  As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, care co-ordination as a Key Worker and delivering evidence based intervention in the community.  You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work flexibly and collaboratively with patients, families/ carers and a range of statutory organisations.

You will have an enthusiastic  approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and the three Advanced Practitioners.

Qualifications

Essential criteria
  • Current professional registration with an approved body
  • Evidence of continuing professional development relevant to post
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards. Qualification must be achieved within agreed timescale.
  • Trained Clinical Supervisor (within agreed timescale).
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
Desirable criteria
  • Post-graduate qualification in a relevant area

Experience

Essential criteria
  • Working with people with mental ill-health
Desirable criteria
  • Working in a person centred therapeutic role
  • Working in a community setting
  • Experience of working with older adult population
  • Working collaboratively with service users and their family/carers
  • Working in a multi-disciplinary team
  • Working in partnership with other agencies

Knowledge

Essential criteria
  • Evidence based knowledge of best practice of caring for service users in designated field
  • Understanding of relevant legislation e.g. MHA/MCA
  • Understanding of safeguarding and it's application in practice
  • Awareness of risk assessment and it's application in practice
Desirable criteria
  • Awareness of psychological models of care and treatment
  • Awareness of research and evidence-based practice in designated field
  • Clinical governance and supervision of other team members

Skills

Essential criteria
  • Communicate sensitive and complex information to service users, carers/family members and all members of the MDT
  • Works effectively as part of an MDT
  • Manage their own time and effective prioritisation of own workload
Desirable criteria
  • Experience of undertaking lead professional and care coordination responsibilities
  • To work towards providing effective teaching, training and supervision of MDT colleagues
  • Competency in ECG and venepuncture or willingness to work towards
