This is an exciting opportunity to join Scarborough Integrated Community team in an newly created development post as a Community Practitioner. The team offer a  friendly and supportive environment where you will be able to learn and develop your skills, providing care and support to the patients in the Scarborough locality. 

The successful candidate will be required to successfully complete a competency framework for 18 months to progress from band 5 to band 6.  The opportunity is open to Nurses and other registered professionals.

The post holder will be required to keep a portfolio of evidence and clinical achievement to support the uplift to band 6. This is a structured process which will be supported through clinical and management supervision.

The successful candidate will be expected to hold a case load as a lead professional/care coordinator and co work with senior practitioners in the team, provide direct patient contact for care planned interventions including any psychological wellbeing interventions/admin of medications as appropriate or within role. To complete triage meetings with trust teams, review referrals with the MDT, complete initial assessments and communicate their needs in professional formulation meetings, care plan and to be able to effectively communicate with partner organisations. You will be expected to be on the duty responder rota and work closely with all members of the MDT.  The successful candidate will also take an active role in providing physical health care of the patients on our case load in conjunction with the physical health lead and medics. Any training required for skills such as phlebotomy or ECG recording can be accessed through the trust. There will also be an expectation that the candidate will support the carers champion role within the team and attend carers support meetings where appropriate.

Interventions with patients may take place in the team building or in their own homes, therefore having access to your own transport is essential to this role.

Qualifications

  • Current professional registration with an approved professional body
  • Evidence of continuing professional development
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards. Qualification must be achieved within agreed timescale.
  • Trained Clinical Supervisor (within agreed timescale)
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
