With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

Job overview

The Enhanced Care Worker  will provide a full range of therapeutic interaction and support to patients, who have complex health and social needs, on a one to one basis whilst maintaining respect for the individuality, confidentiality and dignity of the patient.


They will be work collaboratively with multi-disciplinary team members to ensure a high and consistent standard of enhanced care is delivered to individual patients following the assessment and planning of care by the Enhanced Care Coordinator.


The enhanced care worker will have the opportunity to work  with patients in the enhanced care activity room and on a one to one basis on the ward.

Main duties of the job

To provide Enhanced Care to individual patients who have complex health and social needs within an in patient ward environment or department as directed by a Registered Nurse which includes; 

  • Enhanced observation of patients who are deemed suitable including patients with dementia, learning disability, mental health, drug & alcohol withdrawal, falls, abscond risk, self-harm and delirium.
  • Accompanying patients, allowing freedom to walk, with staff permission
  • Use meaningful and therapeutic activities to engage with patients
  • Talking and listening to patients
  • Assist with nutritional and hydration needs with staff permission and guidance
  • Accompany patients off the ward for appointments with staff permission
  • Accurately record clinical observations of patients receiving enhanced care.
  • Help patients to wash, toilet, and dress using correct manual handling techniques.
  • To provide advice, support and reassurance to patients and their families whilst maintaining their privacy, dignity, confidentiality and safety throughout a patients admission and stay in hospital.

Detailed job description and main responsibilities

Responsibilities:

To provide Enhanced Care to individual patients who have complex health and social needs within an in patient ward environment or department as directed by a Registered Nurse which includes;

 Enhanced observation

Accompanying patients, allowing freedom to walk, with staff permission
Playing cards, jigsaws, drawing etc

 Talking and listening to patients

Assist with nutritional and hydration needs with staff permission and guidance.

 Accompany patients off the ward for appointments with staff permission.

Accurately record clinical observations of patients receiving enhanced care.

 Help wash, toilet and manually handle patients

Clean and decontaminate equipment after use including hoists, commodes etc.

 Answer the ward phone when not providing 1:1 care

 Answer patient nurse call alarms when not providing 1:1 care.

To be actively involved in the daily reassessment of in-patients whom enhanced care has been provided to, liaising with the ECC, ward staff and other health care professionals to ensure high quality enhanced care is provided when required.


 To provide advice, support and reassurance to patients and their families whilst maintaining their privacy, dignity, confidentiality and safety throughout a patients admission and stay in hospital.

 Establish and maintain effective communication and good relationships with patients, carers and staff alike.

To liaise with the ward staff to ensure and support appropriate and timely assessment and planned care plans for patients and clearly document the care provided.

 To only perform duties that sit within the parameters of personal capabilities and competencies.

Participate in programmes of appropriate mandatory and bespoke training as required.

Appropriately escalate any concerns to the nurse in charge.

Adhere fully to the Trusts uniform policy and represent the service in a professional and responsible manner.

To rotate and provide support and care into the enhanced care activity room delivering activities planned by the enhanced care activity room workers.

Support the Enhanced Care Coordinator in the auditing of the service when required.

 Promote a culture of respect, dignity and informed choice for persons with complex health and social needs, through advocacy and support in decision-making.

Communication and Working Relationships:

Ability to use appropriate communication skills including tact, diplomacy, discretion, empathy, negotiation skills, motivational skills, persuading and influencing skills.

 A requirement to communicate and understand information in potentially emotionally charged and hostile situations.

Communicate information to other staff members e.g. Ward based Multidisciplinary members and an Enhanced Care Coordinator.

Most challenging part of the job:

To inspire and motivate, in partnership with patients, carers and staff.

To provide high quality and consistent enhanced care to patients who have complex health and social needs.

 Ensuring that timely, accurate and consistent documentation reflects the care given to patients and the individual needs of that patient at the time.

Qualifications

Essential criteria
  • NVQ level II in care or Level 3 Diploma in Care / Qualified Modern Apprentice in Health Care

Experience

  • Nursing care experience gained within an NHS or care home setting.
