Discharge Liason Facilitator

Twineham, Orthopaedic ward, PRH, Discharge Facilitator Band 4

We are pleased to announce an exciting opportunity for a Band 4 discharge facilitator on Twineham Ward.

Twineham is a 37 bedded orthopaedic ward which provides a dedicated fractured neck of femur service, specialised care and acute rehabilitation for patients following emergency and elective surgery.

Twineham ward is a dynamic environment to work in and through the establishment of the Patient First Improvement Programme (PFIS) it strives to continually improve the services it delivers to benefit both patients and staff.

We also have a well-established and integrated multidisciplinary team, clinical nurse educator and dementia nurse lead. Together the team focuses individually on each case, so we can achieve the best outcomes for our patients with safe and timely discharges.

The successful candidate will need to be motivated, dynamic and innovative while possessing strong organisational and interpersonal skills. This is an exceptional opportunity for a Band 4 who believes they have the right aptitude and desire to learn and develop new skills within orthopaedic speciality.

As the Discharge Facilitator on Twineham ward you will be a key member of the team. You will help to run the MDT meetings, liaise with the occupational therapist, physiotherapists, and community and specialist teams, including social services, community rehab teams, and complete the documentation to refer patients onto the services involved in the planning and implementation of the patients discharge process. You will be involved in the active focus of reducing the length of stay of the patients and facilitating their safe and secure pathway out of hospital.

We will provide a friendly and professional environment for you to work in. Additional training will be provided to support you in the role.
