Job overview

A vacancy for an experienced band 6 health facilitator has become available in the Learning Disability Health Facilitation Team.  Professions other than nurses would also be considered with appropriate learning disability experience. The successful candidate could be based in either Worthing or Horsham.

The Learning Disability Health Facilitation Team support people with learning disabilities to access health care (in hospitals and in the community) within West Sussex.  

Main duties of the job

To provide specialist learning disability knowledge, expertise and input to support access to healthcare for people with a learning disability. To ensure that people with a learning disability living in West Sussex can effectively access primary and secondary health care services as required, resulting in improved health outcomes.

Detailed job description and main responsibilities

To work with clinical staff within the acute hospitals to support and ensure the provision of safe, high quality services for individuals with learning disabilities using inpatient services, minimising risk, incident and error and fostering a culture of learning and reflection.

To support implementation of the Directed Enhanced Service (DES) Annual Health checks for people with Learning Disabilities, through provision of specialist education and support to Primary Care Staff to ensure that practices are able to meet its requirements and to promote uptake of the DES amongst practices across West Sussex.

Qualifications

Essential criteria
  • Registered Nurse Qualification or allied health professional and current entry on a professional register
  • Evidence of ongoing training in specialist learning disability area and commitment to development.
Desirable criteria
  • Knowledge to degree level or equivalent

Experience

Essential criteria
  • Directly working with people with learning disabilities and their carers. Evidence of working in a multi-disciplinary / agency setting
  • Experience of working collaboratively and in partnership with a range of health and social care professionals, individuals with a learning disability and their carers to plan, develop and evaluate services.
Desirable criteria
  • Facilitating and organising training workshops and, developing and using health education materials for people with a learning disability
  • Experience of working with people with a learning disability in acute general hospital settings and undertaking a health facilitation role.

Skills

Essential criteria
  • Ability to be self-motivated, plan and organise own workload, work independently on own initiative with limited supervision and also as part of a team.
  • High standard of written and verbal communication skills including ability to problem solve, present information, negotiate effectively, listen and be empathetic and tactful.
Desirable criteria
  • Ability to present information in more accessible formats and developing Easy Read documents
