We have an opportunity for a Registered Mental Health Practitioner to join our established First Episode Psychosis Service and developing At Risk Mental State service.  The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a First Episode Psychotic disorder: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live.  You will work with a Multidisciplinary Team: a spectrum of clinical professionals that includes Nurses, Psychologists, Support Time Recovery workers, etc.  

This position is suitable for either a qualified Nurse or Social Worker and whilst past experience of working with this service user cohort is desirable it is not essential as training will be provided to the successful candidate.

You will work as part of an established multi-disciplinary team and support future service development.  You will attend the weekly Multi-Disciplinary Team meeting to ensure full oversight of service user needs.

You will act as a Care Co-ordinator for an identified caseload of approximately 15 service users from initial assessment to their transfer to system partners via the delivery of personalized care that is recovery focused.  This will include the completion of comprehensive risk assessments/risk management plans.

You will support service users, their carers and wider support networks by providing education to enhance their understanding of mental illness with the aim of preventing relapse and hospital admission. 

You will oversee medication management by monitoring concordance, efficacy and potential adverse reactions whilst providing education as to the role that medication play in terms of recovery. 

You will work alongside the service leads to ensure all NCAP requirements/Key Performance Indicators are attained and sustained.

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

Act as a Care Co-ordinator:  providing and oversight of the management and care of service users.

Produce accurate and timely documentation in line with your Professional Code of Conduct.

Undertake comprehensive assessments, formulating personalized care plan that reflect the needs of the service user, their family and wider support networks: including carer assessments.

Undertake risk assessments, attend the weekly MDT meetings to ensure whole system has oversight of identified risks and risk management plans.

Provide education to service user, their care network and system partners.

Administer medication, as required: assess for adverse reactions, review concordance and efficacy.

Engage in any required commissioning processes by completing Mental Health Nursing Needs Assessment, Care Act Assessments and contribute to Self-Directed Support assessments/packages.

Participate in service development reviews and NCAP audits.

Work  closely with the Team Manager and Clinical Manager with regards ti meeting Key Performance indicators, future service development and implementation of the Teams Operational policies. 

Act as a supervisor for lower graded staff, as required to do so.

Complete all mandatory training in line with Trust policy and attend in-house development training.

The West Essex Locality has excellent access to both London and Cambridge with the service operating from dedicated sites in Uttlesford, Loughton and Harlow.  These areas cover a range of communities that vary from the more vibrant to the quieter rural setting.
