Job overview

The Band 6 Specialist Diabetes Nurse will be an exciting opportunity, and you will be working with a supportive and compassionate team.  
 
Gloucestershire Hospitals NHS Foundation Trust offers a countywide Diabetes and Endocrinology service in a variety of clinic-based locations. We are looking for a compassionate Nurses to join our team to help deliver high standard of care and engage well within a multidisciplinary team.

You will be working with a fully established  specialist nurses diabetes, dietetic specialist team and Speciality doctors. The lead band 7 Specialist Nurse Diabetes will be supporting with induction and offer regular supervision. You should also be able to evidence consistent development of your knowledge, in order to provide patients with advanced treatment options .  

Main duties of the job

The candidate should communicate effectively and actively participate in Multi-Disciplinary Team (MDT) meetings to aid patient care,  joint treatment sessions, case conferences and specialist interest group meetings.

Established knowledge and skills in the field of diabetes,  utilise diabetes specialist skills and competencies to encourage patient to self-manage, maximize rehabilitation potential, to ensure understanding of medication, diabetes as a life-long diagnosis, long term management of their condition, blood glucose & possible complications.  The candidate should show good leadership skills, be responsible for mentoring junior staff and help them in achieving and maintaining competencies.  

The candidate would be able to manage the countywide outpatients through clinic, including the administrative activities including dictating letters, organizing patient’s follow-ups. Also, to use clinical reasoning skills and techniques to assess, diagnose, plan and offer a range of treatment options; deliver, evaluate and amend the treatment plan, to make recommendations for appropriate and timely management in an outpatient and inpatient setting. Supporting admission to and discharge/transfer from hospital.

Work closely with the Lead nurse, Matron and Clinical lead to ensure the operational delivery of high-quality care, clinical safety & effective clinical governance, ensuring appropriate cross county cover for diabetes nursing care within given resources.

Knowledge and Skills

Essential criteria
  • Registered Nurse with the Nursing Midwifery Council
  • Diabetes Nursing Qualification e.g. Certificate in Diabetes Care or equivalent.
  • Currently working as a diabetes specialist nurse practice.
  • Excellent communication and interpersonal skills
  • Creative and Flexible
  • Teaching / Mentor Qualification
Desirable criteria
  • Non-Medical Prescriber
  • Advanced IT skills that include the use of hospital IT systems and patient portals
  • Ability to switch between different activities and unpredictable demands to prevent hospital admissions and accelerate hospital discharges
  • A person who is able to be flexible, self-motivated and forward thinking.
