Health Care Support Worker Discharge Lounge and Medical Day Case

Job overview

We are looking for enthusiastic motivated keen individuals to join our friendly team to deliver care to our patients.

We are a dual unit incorporating Medical Day Case and Discharge Lounge.

Medical Day Case patients are admitted directly to the department for their regular treatment for their chronic health conditions including specialities from Haematology Rheumatology Gastroenterology and Neurology to name a few thus preventing an admission to hospital as an inpatient.

We also have patients requiring Liver Lung Kidney abdominal biopsies and insertion of ascetic drains which we care for pre and post procedure.

Discharge patients are admitted from all departments in the hospital whilst awaiting transport and medications.

We ensure that they are discharged in a safe and timely manner to their destination on discharge whether it be  a care home community hospital or home address.

You will be willing to :

Learn new skills.

Deliver high standards of care to patients whilst in the department.

Update documentation as required using computer systems.

Supporting the registered nurses to deliver the plan of care.

Collecting patients from all areas of the hospital for Discharge and collecting patients from x ray after the patients have had their procedures complete.

Medical Day Case department is open Monday to Friday 0830-1630

Discharge Lounge is open from 0830-2000 Monday to Friday

1030-1800 Saturday and Sunday

Main duties of the job

•To assist the registered nurse in the delivery of direct and indirect patient care.

•To report any concerns to the registered nurse relating to standards of cleanliness, hazards and patient safety within the clinical area.

•To ensure the registered nurse is regularly updated on patients’ condition and progress.

•To ensure the equipment is clean and in good working order.

•To ensure the environment is clean and tidy.

•To participate with the induction of staff new to the clinical area.

•To minimize hazards in the working area and report any identifiable hazards to the line manager. To adhere to all safe systems of work applicable to the work area.

•To demonstrate effective communication, both orally and in writing, to ensure dissemination of information.

•To maintain confidentiality at all times. •To promote awareness of child protection issues referring to area child protection policy and Trust policy. Attend child protection training in accordance with the policy.

•To promote awareness of adult protection issues referring to the Herefordshire Multi-Agency Policy and Procedures for The Protection of Vulnerable Adults from Abuse. Attend adult protection training sessions in accordance with the policies and procedures.

Education & Qualifications

Desirable criteria
  • A minimum Maths and English GCSE grades 3 – 9 or grades A,B,C,D or equivalent Functional Skills qualifications
  • Willingness to undertake the Maths and English Functional Skills qualifications
  • Diploma level 2/3 in Health & Social Care
  • Willingness to work towards Apprenticeship level 2/3 in Health & Social Care

Skills, Knowledge & Abilities

Essential criteria
  • Ability to communicate effectively in English, verbally and in writing
Desirable criteria
  • Demonstrate knowledge of roles and responsibilities within clinical setting
  • Knowledge of administrative and demonstrable organisation skills
  • Keyboard skills
