Herts Valleys Clinical Commissioning Group is looking to recruit a Complex Case Manager to work alongside the planned and primary care team as part of the system resilience and quality improvement team to support the assessment of patients accessing the non commissioned activity pathway.
Reporting to the Deputy Head of Resilience & Quality Improvement, you will case manage a group of patients accessing the NCA pathways. Supporting the CCG to ensure that patients have clear treatment goals in place and that these are monitored and reviewed regularly.
Successful candidate will be expected to travel widely to specialist care areas as well as support patient flow out of acute hospitals .
We are looking for a candidate with a clinical qualification (NMC, Allied Health Professionals) and with neurological rehabilitation or system resilience / complex discharge experience.
The successful candidate will be a confident communicators who are able to act on their own initiative and work closely with senior managers in the CCG.
You will have up to date clinical knowledge and understand the role of the clinical commissioning group and integration with social care and other providers.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £942 and a maximum payment of £1632 per annum (pro rata where applicable).
