Senior Mental Health Practitioner

An exciting opportunity is available for a Senior Mental Health Practitioner to join the Sleaford Integrated Place Based Team (IPBT). This role is part of a new place-based mental health workforce that is integrated and co-located within Primary Care Networks.

In LPFT we are forging ahead in our community transformation plans that aim to offer a dynamic, localised approach that is responsive, evaluated & tailored to local need, targeting health inequalities. We are focused on helping people towards prevention and recovery, enabling them to live well in their community as much as possible.

We work closely with the local community to design and improve services, ensuring we use the existing assets and work to continually improve and develop these. It offers the practitioner opportunities for creative approaches to working, ensuring the patient remains central in the care and intervention planning. Support and interventions are delivered that are recovery focused and promote independence and resilience and deliver individual needs led, high quality outcomes for patients and carers.

Key elements of the role will include (but not be limited to):

  • Managing a caseload of highly complex/SMI presentation. Collaboratively working with primary care, secondary care, neighbourhood team, local authority, third sector and voluntary and community organisations; to maintain personalised care plans which focus on what matters to the person.
  • Attends core IBPT working meetings as part of an integrated team and works in a collaborative manner with the Neighbourhood Lead
  •  To work as an accountable and autonomous practitioner, providing specialist clinical knowledge and interventions to service users.
  •  Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision
  •  Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
  • To supervise other clinical staff within the Integrated Place Based Team
  • Accountable for planning and evaluating the work carried out by self and Integrated Place Based Team colleagues.
  • The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.

Whilst based within the locality IPBT in Sleaford you will provide mental health leadership and implement clinical processes for the team. In addition, you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care.

Within this role, you would work as a valued member across primary and secondary care and with the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.  This role is unique and gives excellent opportunity for joined up working across services.  This not only ensures appropriate and needs led outcomes for the patients, but also offers the practitioner opportunities for creative approaches to working and ensuring the patient remains central in recovery planning. 

The successful candidate will be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. They will demonstrate adaptability, excellent communication skills and self-motivation.

  • Post-Registration experience of working with adults with mental health needs
  • Experience of working autonomously within a senior position
  • Experience of managing staff and good leadership skills
  • Experience of working across organisations
  • Sound knowledge of the national agenda for mental health
  • Sound knowledge of clinical/ risk assessment and understanding of confidentiality
  • Sound knowledge of community care issues, local health and social structures, functions and boundaries
  • Sound knowledge of the Mental Health Act

Qualifications

  • Degree in mental health and social care or equivalent
  • 1st Level Registration (NMC,HCPC)
  • Evidence of advanced clinical practice at post graduate level to masters or equivalent
  • Evidence of continuous professional development
