Job overview

We have an exciting opportunity for a Band 6 Health Visitor to join one of our forward thinking and innovative integrated 0-5 Intensive Support Teams in Westminster.
You will be an integral part of the Local Authority Targeted Early Help Team and be reporting to and working alongside the Intensive Team Lead to deliver the 0-5 Intensive Support program.

You will have a small caseload of up to 25 families with multiple and complex needs under the supervision of the Intensive Team Lead and Early Help Practice Manager, with the aim of supporting families to facilitate change thus improving outcomes for their children. The work will be guided by Systemic Practice and the Early Help Framework and operating model.
Contributing to the delivery of Working Together to Safeguard Children (2018), including improving health and life chances for children and families and safeguarding those at risk.

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)

Co-located in the Family Hubs Bessborough Street, we’re a unique service integrated with the Local Authority Early Help Team which includes Health Visitors and Early Help Practitioners.


The intensive 0 – 5 programme is a new, early intervention home visiting programme delivered over the course of a year. The underlying aim of the intervention is to enhance parent - child relationships as a means to improving child development and school readiness thereby preventing escalation to statutory intervention.

You will assist the Team Lead to deliver the 0-5 intensive support programme and will have the opportunity to develop high levels of professional skills and practice as well as the ability to work as part of a team.

Detailed job description and main responsibilities

Post holders will be expected to develop and effectively manage the emotional intensity of the therapeutic relationships with clients, deliver and plan bespoke interventions and work intensively within complex family situations to achieve the expected outcomes of the programme. To work in collaboration with other early help, social work and health care colleagues to ensure seamless
You will have a small caseload of families with multiple and complex needs under the supervision of the Team Lead with the aim of contributing to the achievement of the aims set out above. The work will be delivered within an agreed framework and operating model. Recruiting, engaging and retaining families on the 0-5 intensive support programme and following programme requirements. This involves gaining parental agreement to participate and for information sharing as well as explicitly explaining the offer and lead professional role to family.

The focus of all work will be on addressing the underlying issues within families rather than just the presenting concerns as a means of preventing re-referrals. You will develop a bespoke SMART Family Plan collaboratively with each family, together with the engagement of other key professionals, and this will then be the focus of your work. As a part of this you will actively engage and work with fathers and other extended family members.

Apply a good understanding of child development, family functioning and parenting interventions and tools, to successfully engage, assess and support families to make changes to their lives that will have positive outcomes for their children. Specifically, learning how to use a systemic assessment and skills in order to effectively create change for families, mobilise the strengths within families, and move away from a model of case management of “watching and waiting”

Education/Qualification

Essential criteria
  • Registered Nurse/Reg
  • Registered Health Visitor (SCPHN)
  • Current NMC registration
  • Evidence of continuing post-registration education and training
Desirable criteria
  • Practice assessor/supervisor training or working towards

Experience

Essential criteria
  • Health visiting experience  Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets
  • Experience of working collaboratively as part of a multidisciplinary team and inter-agency working
  • Experience of organising and managing own workload and that of others
  • Experience of assessing, planning and implementing programmes of care and writing care plans
  • Experience in providing practice-based learning support to others  Experience of evidence-based care
Desirable criteria
  • Experience of supervising staff / students

Skills and Knowledge

Essential criteria
  • Knowledge of recent developments in primary care  Knowledge of current health and social care agenda relevant to area of work and professional practice
