CLCH have a new opportunity for two qualified Health Visitors to join the inner northwest division and work within either Kensington and Chelsea or Westminster localities. The two boroughs have undergone transformation leading the implementation of universal and targeted teams that are aligned to family hubs and the wider early help programme.

You will undertake the Health Visitor role for 80% of your clinical time. 20% of your role will be dedicated to working within a structured 6–9-month work-based programme working under the supervision of the Public Health Nurse Consultant. You will identify, design and manage a project focusing on health at a population level, looking at ways to make communities and environments healthier, reducing ill-health and tackling health inequalities.

The inner boroughs are a great place to work as you will experience the full spectrum of need from including the least and most affluent  which is why this role is essential in determining the local health need. 

The division is services by 20 TFL stations and 5 cross county/country stations that will help you commute and support work-related travel.  You will have the opportunity to work across areas such as little Venice, soho, the west end, Knightsbridge and more.

Specialist Community Public Health Nurse and Public Health Quality Fellow

To plan, organize and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high quality service

To provide a skilled, effective and evidence based health visiting service to clients in a variety of community settings.

To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities

To collaborate with team colleagues in order to deliver the Quality Strategy objectives

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C’s apply)

To compliment your role as a Health Visitor, you will:

Focus on health at a population level, looking at ways to make communities and environments healthier, reducing ill-health and tackling health inequalities

Measure, monitor and report population health and wellbeing; health needs, risks, and inequalities; for a specific chosen area of your work

Work with the Trust Public Health team and existing childrens services to promote population and community health and wellbeing, addressing the wider determinants of health and health inequalities

Audit, evaluate and re-design services and interventions for a specific chosen area of your work to improve health outcomes and reduce health inequalities

Work with, and through, policies and strategies to improve health outcomes and reduce health inequalities

Work collaboratively across agencies and boundaries to improve health outcomes and reduce health inequalities for a specific chosen area of your work

Provide leadership to drive improvement in health outcomes and the reduction of health inequalities
