Job overview

We are looking for Registered Nurses to join Moorfields Private in our Outpatient Centre. The successful applicant will be responsible for:

-    Protecting and supporting the health of individual patients and clients.

-  The assessment of care and health education needs.

-  The development, implementation and evaluation of programmes of care including  discharge planning for each patient.

Main duties of the job

To maintain accountability for assessing, planning, implementing, and

 evaluating programmes of care within the framework of team/primary nursing.

 To ensure safe discharge of patients.

 To promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies. Help patients gain access to health and social care, information and support relevant to their needs.

To educate patients and their family/carers as required where a need has been identified.

 To provide information which enables patients to make choices about adopting a healthier lifestyle.

 To complete patient documentation correctly, appropriately and in a timely manner.

 To maintain a safe ward/departmental environment and work with other members of the team to produce health care environments that are conducive to safe, therapeutic care and ethical practice.

To assist in establishing and monitoring protocols/care pathways.

To participate in the promotion and maintenance of effective communication.

Detailed job description and main responsibilities

  • To work according to the Nursing & Midwifery Council and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:-
  • Protecting and supporting the health of individual patients and clients.
  • The assessment of care and health education needs.
  • The development, implementation and evaluation of program of care including discharge planning for each patient. To gain experience and skills in:-
  • Clinical practice for Ophthalmic Private  Outpatient service
  • Facilitation and teaching for diagnostics ophthalmic test, procedures  and evaluation
  • Management (as appropriate
  • To work in accordance with the MEH Nursing Strategy
  • To maintain effective communication.

Main responsibilities:

            Patient Care Responsibilities

1.1       (Acting as a Named Nurse with facilitation), to maintain accountability for assessing, planning, implementing and evaluating programs of care within the framework of team/primary nursing.

  1. Professional Development and Education Responsibilities

2.1       To develop own teaching skills and participate in staff/student education programs that develop competence and performance.

2.2       To help maintain a suitable learning environment for staff and act as a facilitator in the supervision and teaching of nursing students and others to develop their competence.

2.3       To attend Trust/local orientation programs and mandatory training sessions and annual updates.

  • To be responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with PREP requirements and to maintain a professional profile.

2.5        To be accountable for own practice and ensure competence in own practice.

2.6       To be appraised at least annually and contribute to personal performance/development plan.

2.7       To participate in clinical supervision, and relevant developmental activities as devised by the Practice Development team.

2.8       To develop computer literacy skills and work towards developing computer skills in relation to E-Patient

2.9       To assist in the training and development of Health Care Assistants and complete assessor’s training as required.

Research and Development Responsibilities

3.1       To assist in promoting nursing practice in line with relevant research and evidence based practice.

3.3       To deliver care based on current evidence, best practice and, where applicable, validated research when available.

  • To contribute to research and development programmes within the ward/department.
  • To assist in the development and implementation of nursing practice guidelines, standards and policies.

Audit, Quality and Risk Management Responsibilities

4.1       To contribute to the setting and monitoring of measurable standards of care and be accountable for maintaining standards.

4.2       To maintain awareness of the national, professional and local quality issues relevant to the delivery of nursing services, e.g. NHS PLAN, Making a Difference, Agenda for Change, Modernisation agenda etc.

4.3       To uphold quality initiatives that improve ‘customer care’ and enhance the interface between staff, patients and visitors.

  • To participate in the audit process for monitoring and reviewing nursing quality and assist in raising the impact of the “Essence of Care” Benchmarking process and implementing the action plans.
  • To develop an awareness of personal responsibilities and assist in the maintenance of a safe environment and identification of potential risks for all personnel, patients and visitors.

4.6       To be aware of the role of the nurse in handling complaints in accordance with MEH policy.
