The North Middlesex University Hospital NHS Trust is a medium-sized acute and community trust with over 525 beds and over 600 community staff, serving more than 600,000 people living across Enfield and Haringey and the surrounding areas, including Barnet and Waltham Forest.

The hospital has been on its present site for over 100 years and was established as an NHS trust by statute in December 1990. The Trust’s services are provided on the North Middlesex University Hospital site as well as a range of community sites across the London Borough of Enfield, including at partner hospitals. They provide services in collaboration with a range of partners, including local GPs, acute, mental health and other community health service providers across North Central London.

In the year ending 31 March 2022, the Trust reported a turnover of £419.7m and employed almost 4,000 staff. Following the transfer of Enfield Community Services on 1st April 2023, this has increased as we have welcomed over 600 new staff including District Nurses, Community Matrons, Community Physiotherapists, Psychologists and many more across a wide range of adult and children’s community services in Enfield. It is an exciting time to join North Mid as we continue our journey to become an integrated care organisation to deliver high quality, seamless care in our local communities, with a focus on tackling health inequalities.

The postholder will  support the Clinical and Nursing Lead for Gastroenterology and Endoscopy, in meeting and maintaining the standards required by the Joint Advisory Group on Gastrointestinal Endoscopy (JAG).  They will lead, manage, direct, and evaluate the nursing care for a defined caseload of patients in all areas of Endoscopy i.e. pre-assessment, admission, procedure rooms, recovery, and discharge.

Detailed  knowledge of Infection control and decontamination of endoscopes. 

Maintaining quality and delivering high standards of care by adhering to trust values.

Weekend and weekdays on-call duties as per rotation.

Supervision of staff and delegation of authority to Junior staff as and when required.

Mentoring and training newly recruited staff and students and live by a great an example.

Competent in health roster and allocating staff with right skill mix.

Duties and Responsibilities:

 Main Duties

  • Supporting Endoscopy Unit manager and officiating in her/his absence. Working in various rotation duties during the week and weekend.

  • Maintaining quality and delivering high standards of care by adhering to trust values.

  • Pre-assessment of patients for complex procedures (inpatients ant outpatients) e.g. EUS, ERCP, PEG, Polypectomies, Capsules endoscopy.

Clinical Role

  • Ensure patients are pre-assessed prior to complex Endoscopic procedures.
  • To be responsible for the assessment, planning, implementation, evaluation of each individual patient’s care, taking into consideration the physical, psychological, social and spiritual needs.
  • To be able to take a full comprehensive medical history present and past including the social history from the patients.
  • Assess the suitability for patients requiring a surgical procedure or operation by providing appropriate routine investigations and assessment.
  • Arrange other specialist investigation i.e. Echocardiogram and Respiratory Function test.
  • Provide support and information to patients and relatives on all aspects of surgery prior to admission, which includes Health Education.
  • To work with agreed Local and National Pre- Assessment protocols and guidelines supported by and in conjunction with the Modernisation Agency.
  • The supervision of junior staff and teaching of qualified/unqualified staff including learner nurses.
  • To lead the team when designated ensuring the maintenance of a high and safe standard of care.
  • To ensure that medical instructions are implemented as appropriate, report fully on patient status to the medical and nursing staff.
  • The maintenance of an acceptable environment for patients in clinic, reporting and taking appropriate action in emergency situations and for personally attending the compulsory update sessions.
  • Reporting to the Unit Manager or Senior Divisional Nurse in the absence to the former, any untoward incidents occurring in the unit.
  • Ensuring that all form of communication is used effectively in the interests of patient care.
  • Carry out full assessment of designated patients and be able to supervise learner nurses and Health Care Support Workers.
  • Liaise with Anaesthetic and Senior Medical Staff to ensure that patients are fit for surgery, arranging alternative management as necessary including referring patients back to their GP or specialists in other professions as necessary.
  • Identify clinical abnormalities that will impair on the surgical journey and act to resolve them.
  • Maintain accurate records including pre-operative testing and care interventions to form the basis of total patient’s records.
  • Ensure work with the Trust/Unit and Pre-Assessment policies in order to maintain professional and patient safety.
  • To undertake Phlebotomy and Cannulation within the clinical area and be responsible for supervising staff undertaking training.
  • Train and monitor good hand washing hygiene, endoscope decontamination and environmental cleaning.
  • Acquire knowledge and expertise to take appropriate action in emergency situations and for personally attending the compulsory update sessions.
  • Ensure a working knowledge of all equipment of all equipment in use in clinical area.
  • Responsible for the monitoring and audit of the pre-operative assessment service against the service objectives. Assesses the effectiveness of patients’ outcomes and maximises theatre utilisation.

Managerial Role

  • To produce the off duty using health rostering, to monitoring sickness and complete return to work as required in the absence of or under instruction of the unit manager.
  • Ensure sickness and absence/time keeping policies are adhered to by all staff.
  • Ensure that the Trust’s induction policy is adhered to for all new members of staff.
  • Ensure that the staffs are capable to undertaking the duties delegated to them.
  • Ensure that all staff is aware of the Health & Safety, Fire and Cardiac Arrest Policies.
  • Liaise with the domestic administration to ensure the unit is cleaned.
  • Ensure that all staff adheres to the guidelines for infection control as laid down in the infection control manual and to liaise regularly with the senior infection control nurse.
  • Work toward achieving personal objectives and assist junior staff in achieving team objectives.
  • To assist medical staff by reporting on patient’s condition from the pre-op assessment and make suggestions to optimize patient for surgery.
  • To receive and contribute information at departmental meetings and audit.
  • To liaise with other heads of departments for their co-operation and support to ensure the smooth running of the unit.
  • To ensure that confidentiality of ‘patient identifiable’ information is always maintained.
  • Ensuring the safe custody and usage of drugs in accordance with established procedures, policies, and laws.
  • Preparing and supervising the preparation of statements and contributing information to the Unit manager on untoward and unexpected situations relating to any legal responsibilities e.g., accidents to staff/patients/visitors, complaints, or losses.
  • Innovate change when appropriate.
  • Implement the Trusts Corporate Governance Strategy regarding meeting the necessary standards of accountability, financial control, patient and public involvement and the delivery of clinical outcomes.
  • To be responsible for the day-to-day management requirements of the pre-operative assessment service in the absence of the unit manager.

 Education

  • To educate patients and their carers to understand the diagnosis, treatment options and their health care needs during and following treatment.
  • To act as a patient advocate in line with the National guidelines and to actively apply in-depth knowledge of holistic/socio-economic and cultural aspects of care to clients within specialty.
  • Participate in teaching and supervision of new and unqualified staff, including medical staff and learner nurses.
  • Advise on the promotion of health and prevention of illness.
  • Maintain own professional/clinical requirements to act as a role model and resource to patients, significant others and staff.
  • Be conversant with current legislation/JAG/NICE guidance for patients requiring gastroenterology care and implement as appropriate.
  • Participate in local/national audits and research relating to Gastroenterology patients.
  • To use information systems to aid regular audit of the clinical role and have an awareness of how information technology can contribute to clinical practice.
  • Lead on audit, quality, and risk management initiatives in supporting gastro enterology services.
  • Utilise nursing research within clinical practice, keep up to date with, interpret and critically evaluate current literature in specialist area and within Professional arena.
  • To ensure evidence-based practice is implemented and updates Trust guidelines to ensure high quality and effective care is delivered.
  • To communicate and update staff on audit/research findings and to initiate any changes to practice, as required.
  • Work with the Clinical and Nursing Lead to agree own learning objectives in order to develop both professionally and personally.
  • To undertake clinical supervision and reflective practice and support other staff to do the same.
  • Develop and maintain a professional portfolio, which complies with NMC recommendation and revalidation.
  • To monitor/maintain a system for monitoring of self about the requirements of professional practice & the maintenance of NMC requirements regarding registration.
  • To provide information and function as a resource for advice to GPs, primary care about patients requiring endoscopy with both written and verbal correspondence.
  • Participate and contribute with staff review and appraisals and support Personal development plans for nursing and allied health professionals within Endoscopy.
  • Act as an assessor/mentor for students and colleagues participating in induction programmes, teaching, developing and assessing competencies and support ongoing learning.
  • Lead on the development and revision of patient information for patients undergoing endoscopy and ensure all information is in line with Trust standards and agreed within Trust Patient Documentation group.
