We are a Community Mental Health Team (CMHT) providing home based assessment and mental health treatment to Older Adults (over the age of 65) residing in the borough of Lambeth. 

 

The team is multidisciplinary in nature with a variety of professionals including Community Psychiatric Nurse's, Occupational Therapists, Psychiatrists, Psychologists, Clinical Support Workers and are co-located with a specialised older adult mental health social work team. The Lambeth Older Adults CMHT is dynamic and has a supportive culture and recently in recent staff surveys were identified as one of the most engaged teams across the Trust. 

We offer culturally informed assessment and treatment within a biopsychosocial model to people (and their families/carers) with a range of different mental health needs, including:

  • Mood disorders 
  • Anxiety and stress
  • Dementia or memory loss - we also work with people under age of 65 with progressive memory problems
  • Other mental health problems to do with mood or mental well-being which may affect daily living.

We are looking for a dynamic Community Psychiatric Nurse or Occupational Therapist to join our team as a care coordinator. Although the role is largely care coordination there is scope for discipline specific roles, tasks and development.

An exciting opportunity has arisen in Lambeth CMHT for a Community Psychiatric Nurse or Occupational Therapist. 

You will manage a caseload of clients with complex mental health needs as part of the Care Program Approach (CPA) as well as support our duty and triage function.

The post holder will be expected to work autonomously as part of the dynamic team, and be able to formulate flexible, robust and holistic care plans with clients and/or carers that aim to guide service users towards reaching an optimum level of functioning and support them to maintain good mental health.

Qualifications

  • Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse or registered Occupational Therapist
  • Mentorship qualification or equivalent experience mentoring students

  • Relevant Master’s degree

Skills and Abilities

  • Skills in clinical assessment/baseline mental health assessment
  • Skills in risk assessment and risk management and crisis management
  • Able to provide quality care that is responsive to service user’s needs, without close supervision
  • Self-aware, self-confident and intrinsically motivated to do a good job and to motivate others
  • Experience working in a MDT setting

Experience and Knowledge

  • Evidence of continuing professional development
  • Knowledge of relevant Professional and Clinical legislation
  • Understanding of Care Co- ordination under CPA
  • Experience working with older adults
