Job overview

The post holder will lead a highly skilled team of community nurses to provide excellent Nursing care and support for patients during the evening and night time.

The role also manages the Single point of access triage team which operates 7 days a week

 This is an exciting opportunity to gain experience  in District Nursing at Team leader level covering Maternity leave  ( 9 months temporary)

Current  post is Band 7 District Nurse Team Leader it is a  full time 37.5 hours per week within our Out of hours service and also single point of access triage team and Palliative care Respite Team.  You will need to be a Specialist Practitioner and experience of working at Band 6 level within District Nursing.

Main duties of the job

Our work in Primary Care Networks is at the heart of Stockport

We are looking for a Band 7 Nurse who is flexible, innovative practitioner with a variety of clinical and leadership skills. The post is full time , based at Kingsgate House Stockport leading the Out of Hours District Nursing service and our Single Point of access triage team. This post involves out of hours working & being part of a contemporary , skilled team.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £43.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

Detailed job description and main responsibilities

The post holder is responsible for the assessment of care needs, planning, implementation and evaluation of programmes of care for patients Out of Hours, exercising a higher level of clinical judgment, discretion and decision making. The post holder is responsible for the  leadership of a team of community nurses.  You will play a key role in demonstrating innovative practice, service improvement and maintaining the continued professional development of team members and learners in the clinical environment.

Education & Qualifications

Essential criteria
  • 1st Level Registered Nurse
  • Community Specialist Practitioner District Nursing First Degree Working to masters degree
Desirable criteria
  • Nurse Prescriber Teaching qualification

Experience

Essential criteria
  • subtantives years experience in the management of a team and staff in a community setting. Either at Band 6 or 7 level
Desirable criteria
  • Experience in the management of Human and financial resources including unexpected fluctuations in workload and/or workload
  • LEAN Methodology
  • Experience of managing change/innovation Experience in clinical governance and its implications for services.
