An exciting opportunity has arisen to join our well established Teletriage team.

Teletriage Nurse Clinicians work within the Community Transfer of Care Hub (ToCH) aligning with the National Framework for Urgent Care Response (UCR) 2 hr response

Their role is to facilitate  high quality clinical assessment of acutely unwell patients within the care home setting, and support the frailty initiative by providing senior clinical support to ward staff within the Community Integrated Care Centre (CICC) in order to reduce readmission back to the acute hospital 

 

Main duties of the job

The post holder will utilise established pathways and processes to provide a wide range of nursing and social care assessments to patients who may potentially be at risk of a hospital admission. The role will involve supporting acute ward, CICC, Nursing and Residential care home staff in determining the best pathway of clinical care for the patient.

The post holder will be an autonomous practitioner who has the experience and clinical confidence to assess the needs of the patient and determine if the patient needs further face to face assessment or whether they can be discharged at first point of contact.

Detailed job description and main responsibilities

The primary aim of the service is to prevent inappropriate admission to hospital. To support and educate acute wards, CICC, Nursing and Residential home clinicians to make the correct clinical decision for their patients and residents.

The main responsibilities of the role will include

  • assessment, planning, implementation, and evaluation of programs of care, which meet the needs of patients
  • To provide advice, information and support for doctors, nurses and social workers across hospital and community settings, regarding the implementation of patient care
  • Responsible for the initial holistic assessment, implementation, and evaluation of programmes of care with often complex needs of acutely and chronically unwell patients within the Community setting without supervision.
  •  Make clinical judgements and decisions that can often require analysis of complex facts/situations that demand prompt action
  •  Work within a multi-disciplinary team with support workers, physiotherapists, occupational therapists, community psychiatric nurse and social workers. Providing integrated skills and sharing knowledge.

Qualifications and professional training

Essential criteria
  • RN qualification
  • Masters in advanced practice, or working towards
  • Evidence of recent Professional Development
  • Teaching and Assessing Qualification or equivalent
Desirable criteria
  • Experience of working in Urgent Care
  • Independent Prescriber
  • Post Graduate qualification in rapid response/community nursing/emergency department/acute medical or equivalent experienc

Experience and knowledge

Essential criteria
  • Minimum of 3 years post registration experience
  • To have an up to date working knowledge of acute, chronic, and terminal illness, wound management, care of the elderly and Palliative care.
  • An understanding of the Primary and Secondary Care Interface
  • Demonstrate the application of evidence-based practice.
  • Able to demonstrate a depth and breadth of current issues and policies within the NHS and can translate into practice
  • Autonomous practitioner working independently and in conjunction with other health care professionals
  • Experience in leadership role
  • Evidence of continued professional development
  • Experience of Clinical Governance and risk assessment
  • Experience in end of life care
  • Experience in clinically managing patients both face to face and over the telephone
