Job overview

The Acute Ward at Great Yarmouth and Waveney Acute Services are excited to advertise for a Band 6 Physical Health Nurse to work with the team on the ward.

The purpose of the role is to improve the physical health care of mental health service users in accordance with directions and standards issued by the Physical Health Team based on national guidance.  

The post holder will act as an expert resource within NSFT by working in collaboration with members of the physical and mental health team.  To work with clinicians of all disciplines providing advice and guidance to support the effective and safe delivery of the physical health agenda.

Main duties of the job

The purpose of the role is to improve the physical health care of service users under the care of Northgate Acute Services in line with Trust Policies, Strategy and National Guidance.

The Physical Health Nurse is employed to:

Provide a wide range of physical health monitoring and brief intervention treatment services to service users whilst they are on the ward.

Train and supervise other Trust staff: nurses, clinical support workers and medical staff on clinical and physiological observations, new equipment, monitoring tests and physical care interventions.

Recognise changes in routine physiological observations and escalate these to the relevant referral route to enable efficient management of illness and disease.

To work with the Lead Physical Health Nurse to provide ongoing disease management through use of co-produced care plans.

Provide advice on smoking cessation, weight management and sexual health initiatives to service users.

Assist with maintaining a comprehensive network of link persons from within and external to the Trust and ensure best practice is communicated and cascaded to the team, supporting the Trust and NHS initiatives that enhance the wellbeing of service users.

Relay guidance to Trust staff on the safe use of medical devices and monitoring equipment (including assisting with selection and teaching use).

Support the team with audit processes relating to specific monitoring of the service provisions on the Acute Ward.

  • Post registration qualification in health promoting smoking, sexual health and well person screening

Experience

Essential criteria
  • Experience of developing, delivering and evaluating training particularly around behaviour and lifestyle
  • Experience in delivering health promotion/ public intervention
  • Long term conditions, wound care monitoring to prevent deterioration
  • Experience in completing and reviewing audit data
Desirable criteria
  • Experience in Mental Health

Skills

Essential criteria
  • Able to teach ECG application, Phlebotomy, Physiological monitoring procedures
  • Must be able to use computer and perform basic word processing
Desirable criteria
  • Willingness to study these topics

Knowledge

Essential criteria
  • Knowledge of public health and NHS improvement initiatives
  • Well person screening, physiological monitoring tests and selection of interventions or referral options for wound care, respiratory illness such as COPD, diabetes, sexual health and family planning
Desirable criteria
  • Recognised accreditation in Quality Improvement
