Job overview
An exciting opportunity is available for an experienced Mental Health Practitioner to join our MSNAP Accredited Memory Assessment Service. They must have either completed their Independent Prescribing Course or are currently undertaking the course. Experienced Band 6 nurses, with ambitions to develop the competencies needed to achieve B7, including completing the prescribing qualification, should consider applying as part of a developmental role.
The Memory Assessment Service East (CMAS) is clinician led with a strong emphasis on multi-disciplinary team working to ensure that users of the service receive high quality, compassionate care throughout their memory assessment process. You will work within a full MDT which includes: nurses, OTs, support workers, a Consultant Psychiatrist and Clinical Psychology. We are supported by a strong admin presence.

The team is currently embracing the opportunity to be part of a Suffolk-wide Memory Assessment Service offer wherein services in both East and West Suffolk are aligned and accredited to the same standard of quality of care. A rebrand of these services over the coming year as the Suffolk Dementia Assessment Services is currently in the works, making this a fantastic time to be part of the development of this ambitious new plan.
Main duties of the job
General responsibilities include:
- Having a good understanding of dementia subtypes.
- Completing holistic assessments to investigate cognitive changes and symptoms of dementia. Whilst using a variety of cognitive assessment tools and being able to interpret the results.
- Attending a weekly Diagnostic Formulation MDT to formulate diagnoses.
-Ability to sensitively deliver a dementia diagnoses.
- As an Independent Prescriber, you will be responsible for initiating treatment as agreed by the MDT and in line with National Guidance. As an Independent Prescriber, you will be responsible for organising follow-ups and reviews of service users throughout the titration process, monitoring drug efficacy and tolerance. Most of these contacts are made in clinical areas, though some home visits are accommodated as required.
- Supporting with a duty role, and the screening and triaging of incoming referrals.
- There is expectation that support will be provided to students on placements as a practice assessor and/or supervisor.
-Supporting the facilitation of post-diagnostic groups.
- Attending monthly Management and Clinical supervision as well as providing managerial supervision to colleagues.
- Participation in research projects.
- Development of robust relationships with partner agencies and services.
- You will be expected to be able to work autonomously and manage your time well. You must be able to be able to travel to various locations across Suffolk.
