Physical Health Assistant Practitioner

Norfolk and Suffolk Foundation Trust are on an improvement journey with a focused sense of direction. This is where you come in! We have an exciting opportunity for a 6 month secondment for a Physical Health Assistant Practitioner to join our Samphire Ward team to support the assessment and monitoring of the physical health of people with Severe Mental Illness in accordance with NICE guidelines.

Samphire Ward is a 16-bed acute admission ward housed in a purpose-built facility built in consultation with both staff and service users.

We want ambitious people to apply for this role and we take a creative outlook on staff development.  Through listening to your values and opinions, the team supports ongoing learning both internally and externally.

You will be responsible for providing and documenting comprehensive physical health assessments, signposting individuals to services to improve their physical health and set goals with individuals to help improve their physical health. This is because it is well recognised that people with SMI have a lower life expectancy than those in the general population, often due to avoidable physical health conditions.

You will be working both autonomously as well as part of the wider Samphire Ward team.

As well as measuring blood pressure and pulse, this will also include more advanced assessment skills and interventions including phlebotomy electrocardiograms and smoking cessation

 

Main duties of the job

If you have a background of working in physical health, hold a foundation degree or equivalent, are passionate about supporting individuals with severe mental illness and understand the importance of the physical health of those with SMI then this could be the position for you.

The position is with an opportunity for clinical growth and development and our team are committed to support you to achieve your potential in this role. You will also be supported and guided by the Trusts Physical Health Team.
