Are you a mental health nurse who is looking for an opportunity to help us develop a new service offering specialist interventions for people with complex psychosis in Suffolk?

Our brand-new community rehab service is seeking two experienced mental health nurses to join their expanding service.

Our MDT is based at Ribbons Park, Ipswich but as the service expands there could be flexibility in the future with a second base in Bury St Edmunds. The team so far is made up of an AHP Lead, Peer Support Worker, Occupational Therapist, Psychologist, dedicated Administrator and Psychiatrist. We also have close links to the inpatient Suffolk Rehab team and a Matron working across both the inpatient and community teams. We have a joined up approach across the Trust and NSFT is committed to developing rehab services across both Norfolk and Suffolk – which means we are working closely with Norfolk colleagues too.

Working as part of our MDT, you will help to shape the new service providing nursing expertise and skills.  In line with NICE guidelines the aim is to help service users work towards personalised goals and managing their own medication and physical health.  

 

Main duties of the job

The role will involve working closely with community partners such as supported accommodation providers and vocational services, as well as our inpatient rehabilitation and recovery service.

Initially you will be offering rehabilitation interventions for people that are case held by the other CMHTs in the Ipswich IDT catchment area.  When the rehab team is fully established the aim is for clinicians to manage a case load of approx. 15 people with a team approach.

As a nurse within our team you will:

·       Carry out assessments, interventions and treatment for people with complex psychosis

·       Opportunities for group work and 1-1 intervention

·       When the service is fully established take on care coordination of a caseload of 15, whilst supporting a shared team caseload approach.  

·       Use of Dialog + to identify meaningful goals 

·       Leading on interventions around self-management of medications

·       Supporting people to engage in activities to develop daily living skills

·       Maintaining patient records, risk assessment and care planning.

·       Medication management

·       Upskill in family interventions and interventions relevant to complex psychosis and rehab

**This post attracts a recruitment premium of £3,000 (pro-rata for part time roles).  This is a one off payment with half paid on appointment and half paid after the completion of a satisfactory probation period (6 months).  The premium is taxable.  Please note that should you leave employment, or choose to move to an alternative post within the Trust that does not attract this recruitment premium, you will be required to reimburse all or some it, as follows:  100% of the value of the premium in the first 12 months; 50% of the value of the premium in the period 12 months to 2 years following appointment.  Please note this recruitment premium is for external applicants only.
