A new role has become available with the Wiltshire Dementia Service.
As a care co-ordinator in the team you will be responsible for a caseload of service users with a diagnosis of dementia, working within the CPA framework to deliver a range of health and social care interventions. You can expect to receive excellent supervision, and there will be opportunities for career development and training in order to support you in this role.
This post is based in West Wiltshire, however as the service is Wiltshire wide there may be a need, on occasion, to travel within the county.
Main duties of the job
To undertake the full range of activities required to deliver ongoing comprehensive mental health assessment for service users living with dementia, living in their own homes in the community and in a range of settings. This will include:
To act as care coordinator.
To plan, deliver and evaluate interventions.
To support carers
To develop and maintain good partnership working with other services throughout all treatment episodes.
To personally build and lead others to build, hope inspiring relationships with service users.
To be responsible for maintaining own and others workload.
To be responsible for the protection of individuals from abuse and harm in line with local safeguarding policies and procedures.
Undertake active clinical leadership and supervision within the team.
To maintain accurate and timely record keeping.
To participate in management, caseload and clinical supervision.
To provide mentoring/training for others.
Demonstrate responsibility for developing own practice in line with professional qualifications and for contributing to the development of others.
Monitor and maintain health, safety and security of self and others, undertaking assessments and taking appropriate action where required.
Develop own knowledge and practice, and contribute to the development of others, making use of available feedback, supervision and appraisal
Lead, maintain and participate in practices which enable effective team working

Detailed job description and main responsibilities
The team is a supportive, welcoming, compassionate team who are client focussed. The team consists of, Nurses, Support workers, administrative team, Consultant Psychiatrists, Psychologists, Occupational therapists etc
The service operates 0900 to 1700 Monday to Friday - closed on bank holidays.

Essential criteria
- Current Full Registration with NMC (Mental Health)
- Clean driving licence and access to a vehicle for work purposes
Desirable criteria
- Experience of working with CPA process
- Previous community mental health nursing experience
- Evidence of extensive experience in relevant client group
- Knowledge of dementia
Applicant requirements
You must have appropriate UK professional registration.
