We have an exciting opportunity for a trainee Advanced Clinical Practitioner (tACP) to work in our Integrated Frailty Crisis Pathway, this role will initially sit within our inpatient advice and liaison service (iOPAL) and perioperative frailty team providing advice and guidance for patients with frailty under different teams within the acute Trust. We welcome applications from both Allied Healthcare Professionals and those with a nursing background who meet essential criteria.
Within the Royal Surrey Frailty services, we are expanding our ACP workforce to help deliver excellent care to all older people with Frailty. We have several established posts within the Acute Frailty Team (AFT), Frailty Same Day Emergency Care (FSDEC), Urgent Community Response (UCR) and Hospital at Home (H@H). These posts contribute to the weekend rota for H@H. With time we hope to be able to rotate trainee ACPs through several services to give them a broader training experience.

You will be required to work as a semi-autonomous clinician within the service and will be supported in your training through our ACP programme aligned to the Frailty Academy. You will have a dedicated educational supervisor as you work towards your MSc in Advanced Clinical Practice. You will be supported in your development as you gain new knowledge and skills aligned to the 4 pillars of Advanced Clinical Practice.
We provide a 7-day service and it is likely you will have to contribute to weekend and evening rotas.

The post holder is an experienced nurse, therapist, pharmacist or paramedic qualified to Degree Level. Individual training needs for this role will be met by undertaking an MSc in Advanced Clinical Practice and a Clinical portfolio in Frailty in order to achieve the level of competence required by the post. The trainee’s pay will be at Band 7 until completion of all competences deemed necessary for autonomous practice where they will then paid at Band 8a.

- NMC or HCPC registered professional
- Post registration specific courses supporting unplanned care assessments
- Eligible for entry onto MSc programme (health related honours degree or a minimum of 90 undergraduate degree level credits – of which at least 15 credits to have been completed in last 2 yrs).
- Higher level qualification in accredited course (e.g. ALS

- Advanced assessment skills qualification with evidence of clinical practice.
- Registered independent non-medical prescriber.
- Teaching qualification.
Knowledge and Skills

- Currently working in a senior role in Therapy (band 6 minimum).
- Senior clinical leadership experience (band 6 minimum).
- Significant post registration experience in unplanned Therapy
- Experience and evidence in developing practice
- Experience of supporting learning in the clinical environment.
- Acknowledged expert in their field who could be clinically credible.

- Experience of participating in clinical audit
- Experience of disseminating work through presentation/publication.
- Previous experience in a practice development role or formal teaching experience
