Are you looking for a new challenge where you can make a difference in the lives of vulnerable young people aged 8-16 not attending school?  To assist them to engage in education or training, access support from appropriate services and ultimately achieve and maintain their independence in the community.

We have an exciting opportunity for a Nurse, Occupational Therapist or Social Worker to join Surrey & Borders NHS Foundation Trust (SABP) in the newly transformed Reaching Out service as part of the Assertive Engagement Pathway. Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network.

We are looking for a clinician who is creative, highly motivated, shows initiative and is passionate and committed for ensuring high quality services for people with mental health needs.

There is 1 full-time post covering the county of Surrey with SABP bases across the county.

Prif ddyletswyddau'r swydd

To manage a caseload of young people aged 8-16 referred to the service and provide mental health assessments. This role will involve outreach into the community to meet young people in their own environment.

To build a trusting relationship to develop a deeper understanding of the needs of young people and understand the barriers in accessing education/training. 

To assist young people to engage in education/training, access support from appropriate services, and be able to signpost or refer to more appropriate services.

To work in partnership with young people, families/ carers, Surrey County Council Area Case Review and Area Action Group and other relevant agencies regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.

Experience

  • 2-year post qualification experience of working in the community with young people with mental health difficulties and their families
  • Experience of and ability to undertake, mental health assessments, risk assessments and implementation of risk management plans
  • Use of outcome measures to evidence good practice
  • Demonstrable expertise working with young people aged between 8 and 16
  • You must have a UK driving license and access to a car for work purposes

Qualifications
