Main duties of the job

To work as a band 6 in a 11 bedded acute frailty unit with an additional 4 bedded frailty SDEC.

To lead the team clinically to deliver excellent care to the specific needs of frailty patients.

To ensure that trust values are met in the delivery of care within this frailty unit. Have an understanding of the needs of frailty patients.

Have an understanding of the value of a frailty SDEC and support in the delivery of the care of these patients.

Clinical excellence.

Understanding of trust policy and procedure and ensure adherence to this.

Detailed job description and main responsibilities

  1. Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies always.

 

  1. Effectively communicate information regarding clinical decisions, policy and care pathways to patients/carers and the multi-professional team including situations where difficulties in comprehension occurs.

 

  1. A skilled practitioner and clinical leader providing support to the management team, participating in service/policy development.

 

  1. Responsible for clinical assessment of a group of patients and management of a team, ensuring appropriate care is planned, implemented, and evaluated/audited, involving patients. and users

 

  1. Organisation of cross-departmental/organisational activities including HR resource, and bleep-holder responsibilities as appropriate

 

  1. Demonstrate highly technical and/or practical skills to ensure optimum patient management including complex patient monitoring; equipment management and frequent administration of invasive tests/procedures

 

  1. Develop and educate staff, participating in supervision, mentorship and appraisal as require
  2. Utilise and ensure others utilise IT systems to secure accurate and timely patient, workforce, and resource data available.
