Job overview

This is a new specialist Physical Health Team expanding and developing within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation Trust.

The Trust is currently undergoing a Community Transformation Programme to improve the physical health outcomes for our patients with direction and standards issued by the Care Quality Commission, NHS England and NICE guidelines.

You will undertake cardio-metabolic monitoring in Out-patient Clinics and complete home visits.

Main duties of the job

The role of Band 5 Clozapine & Physical Health Nurse is to run Out-patient Clinics and home visits to undertake annual cardio-metabolic physical health checks for patients on Clozapine, Lithium, or High-dose Antipsychotics. You will monitor physiological measurements, complete ECG’s and take bloods (training will be provided). You will support a one-stop Clozapine Clinic, ensuring correct monitoring for all Clozapine patients; blood analysing, physical observations and side effect monitoring. Clozapine CPMS training will be provided.

We are seeking skilled, reflective and adaptable clinicians (RMN or RGN) who are able to form good working relationships and will further embed best practice, ensuring parity of esteem to adults with SMI difficulties.

You will be flexible, creative, adaptable and will be committed to working in partnership with patients, promoting the independence and recovery working alongside the MDT and primary care network. You must have excellent communication skills and a genuine interest in improving the physical health for patients. You should be able to form good working relationships with the multi-disciplinary team to deliver a safe, high quality and effective service. You will provide support and supervision to other members of the team.

This is a development post, supervision and training provided to enable you to complete the Trust competency framework. On successful completion, following a min of 12mths the Band 5 will be uplifted to Band 6.

Detailed job description and main responsibilities

  • To provide support, advice, care management and structured interventions to service users who present with physical health issues secondary to severe and enduring mental health problems within a team based approach.
  • To provide a high standard of delivery of care within the Physical Health and Clozapine Clinics including the monitoring of patients on Lithium and High dose antipsychotics
  • To work under supervision of a Senior Nurse, as a professional member of the nursing team within the Core Community Service setting.
  • The post holder is responsible for the assessment of care needs development, implementation and evaluation of programmes of care and will be required to teach nursing and non-nursing staff including basic and post basic students.
  • The post holder will assist the Clozapine and Physical Health Team Manager in developing and maintaining the quality of the department, including monitoring high standards of client care and developing areas of clinical excellence / best practice and a therapeutic & caring culture.

  • Experience of working with people within area of speciality

  • Awareness of the importance of the promotion of physical health care with patient group
  • Trained or willing to undertake the training in Venepuncture
  • Trained or willing to undertake POCHI training
  • Trained or willing to undertake ECG monitoring
  • Commitment / Genuine interest of client group within area of speciality
Desirable criteria
  • Full Driving Licence
  • Ability to support and supervise staff
  • Knowledge of Clozaril / Lithium and high dose anti psychotic medication monitoring
  • Experience of measuring and recording physiological measurements in routine presentations with service users
