Job overview

A vacancy has arisen for a Community Dementia Support Worker within the Dementia Assessment Service (DAS) based in Western West Sussex. The role involves supporting the DAS team in delivering excellent service to the patients under our care.

The successful candidate will support initial assessments in a clinic setting, undertake post-diagnostic clinical review, including time limited interventions helping to aid adjustment to the diagnosis of dementia. They will be offered training to undertake the role to a high standard, individual and group supervision and further training and development opportunities to support them in this role.

This is a well-established team with an extremely low turnover of staff and a high degree of job satisfaction. Roles within this team do not arise regularly so if you are passionate in supporting people to receiving a person centred, timely and meaningful diagnosis of dementia, please don't hesitate to apply.

For those candidates requiring a VISA to work in the UK, please be aware that significant knowledge and experience of at least 2 years, (preferably within the NHS) is required to meet the pay threshold for a Health and Care Visa.

Main duties of the job

­­­­­­­­­­­­The successful candidate will be supported to deliver the following:

  • Support medical staff in clinics by conducting information gathering to aid the assessment process.
  • To carry out post-diagnostic reviews either by telephone or in person and liaise with a qualified member of the multidisciplinary team following the review.
  • Under guidance of the multidisciplinary team, in a time limited manner and where care planned to do so, carry out clinical visits to offer 1:1 intervention to aid adjustment to diagnosis, management of memory difficulties and engagement in meaningful activities to improve quality of life. This will involve visiting service users in their own homes and in other community settings. 
  • To provide people under our care information to help them meet their own needs and signpost to other community services that will assist them in remaining independent.
  • To liaise with other professionals and agencies based on an individual’s needs.
  • There is also the potential for opportunities to help with the administration, organisation and co-delivery of dementia specific groups.
