The post holds many development opportunities and offers a chance to manage an autonomous caseload and plan care around patient centred goals
Main duties of the job
The successful candidate will work the hours over 5 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is home visit / clinic based. You will also work on the ward providing stage 0- 1 of the commissioning pack pathway & in community exercise classes.

- Two years post registration experience, 1 of which must be in recent CVD care
- Understanding of the Commissioning guide for Cardiac Rehabilitation

Qualifications
Essential criteria
- • Registered Nurse (Level1)
- • Current NMC registration
- • Relevant experience / qualification or training in CVD
- • Relevant teaching and assessing qualification
- • Car owner
Desirable criteria
- • First Degree (or working towards)
- • Independent Nurse Prescribing qualification (or willingness to work towards)
- • Experience of working within primary care.
- • Experience of Cardio exercise programmes.
- • Cultural awareness, experience of working with ethnic minority groups
- • Knowledge / understanding of Primary Care
- • Experience of previous service development

You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
