Job overview

The Calderdale Bladder & Bowel Service within CHFT includes the provision of complex bladder and bowel care.

 

The service sees children and adults across HRI, CRH and Calderdale Community. The team provide evidence-based triage, diagnosis, treatment and review of patients with complex continence needs.

The clinical team works as part of a wider MDT. The team provide support and advice to the wider public on continence dysfunction. Care is delivered in a wide variety of health and educational settings

This vacancy is to work as a specialist children’s nurse within the integrated Bladder & Bowel Service, to perform specialist clinical assessments and develop management care plans for complex bladder & bowel conditions within a community setting for children age 4 and above

Main duties of the job

  • The post holder will work autonomously within the team and manage their own caseload of individuals between the ages of 4 – 18 years.
  • The postholder will be responsible for the assessment, management , implementation and evaluation of evidence based programmes of care for children, young person and their family within the community environment.
  • To provide nurse led services for children with complex bowel and bladder needs.
  • Work within the specialist team to ensure a smooth transition for teenage and young adults into adult services.
  • Liaise directly with the multidisciplinary team and other agencies, including education, to facilitate seamless high quality care to the child and family The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice reflects current national and international evidence based theoretical and practical knowledge. The post holder will contribute to the provision of education, advice and support to other health care professionals, and members of the general public.

Detailed job description and main responsibilities

  • The post holder will be involved in teaching carers and families how to manage any treatment, management or incontinence containment programmes.
  • Demonstrates a detailed theoretical and practical knowledge of common aetiologies, pathologies, conditions and presentations – involving an understanding of theories and principles relating to bladder and bowel ·
  • As an autonomous practitioner provide skilled advanced bladder & bowel care to children and their families in a variety of community settings · To work as part of a multi-disciplinary team linking to key personnel in the acute settings, educational organisations and other external agencies key to the child’s health
  • To take full responsibility for a clinical caseload and deliver complex bladder and bowel care to children and their families · Work with patients, carers, schools and families to enable them to develop self-care skills with respect to continence management. ·
  • To act as a key resource for colleagues in developing an increased awareness of bladder and bowel dysfunction · Initiate and follow up referrals where appropriate to relevant clinicians and or social services. To work in collaboration with and be an active member of the MDT including community teams, acute services, social services, and Allied Health professionals to ensure streamlined care

Essential criteria
  • RN Child
  • Comprehensive experience working at a Band 5
Desirable criteria
  • Evidence of interest in and awareness of bladder, bowel function and dysfunction in children

Knowledge

Essential criteria
  • Demonstrable experience of working with children, young people and families
  • Recent experience of working as part of a multi-disciplinary team
  • Evidence in training or an interest in training in issues affecting children and young people’s bladder and bowels Competent in the use of IT to enable mobile working
  • Competent in the use of IT to enable mobile working
  • Experience in caseload management

  • Recent experience of working as part of a multi-disciplinary team

Communication

Essential criteria
  • Excellent communication skills enabling effective communication with other professionals and the ability to contribute to the MDT
  • Problem solving in relation to clinical assessment and care management planning

  • Active team member keen to work as part of a specialist team taking an active role in team development, staff meetings etc
  • Must be eligible to work in the UK. E
