Job overview

Within Hammersmith and Fulham (H&F) CLCH provide a range of services including Health Visiting and School Nursing, Children’s Therapies as well as numerous adult services, this means that you will benefit from a close working relationship with peers and a joined-up approach to care leading to improved health outcomes for the people you care for.  
Within your team you will benefit from a Borough lead with a wealth of experience in Children’s health, Safeguarding, School Nursing and Immunisations and a team that has in excess of 300 years’ experience who are ready to support your learning and development. The team in Hammersmith are based within Parkview Centre for Health and Wellbeing, Richford Gate, Park Medical centre, Parsons Green Health Centre and provide services in local 0-5yr settings.

CLCH believe in helping people achieving their full potential, the team will support to fulfil your role and once settled can support to complete the Nursing Associate, Registered Nurse and Health Visitor qualifications whilst being employed and paid by the organisation.    
The borough has great transport links internally with 3 stations, great bus links across the patch and a full east-west Safer Cycle Pathway route. For commuting you will benefit from access via the Piccadilly, District, Circle and Hammersmith and City lines.  As an NHS employee you will have the option to obtain a lease car or electric bike via the salary sacrifice scheme and have access to a season ticket loan.

Detailed job description and main responsibilities

To deliver the role specific elements of the Universal Healthy Child Programme (DH 2009). Support the work of the 0-19 Public Health Team with planned programmes of public health care and health promotion in accordance with The Healthy Child Programme (DH 2009). Undertake activities as delegated by the Health Visitor, in support of the delivery of the 0-19 service. Support the delivery of a ‘child focused service’ which promotes good partnership working with parents, nurseries, children’s centres and other agencies. To participate in service development and health improvement activities. To monitor, maintain and develop the service. To act as the link 0-19 professional for identified PVIs in line with local service specification
