Job overview

This is an opportunity for an advanced practitioner but would welcome applications from candidates interested in a developmental role.

 37.5 hours per week available.

We are seeking a highly motivated, enthusiastic confident individual who holds Advanced Clinical Practitioner skills or is working towards achieving a Clinical Practitioner Masters to join our team. You will work within a dynamic team of clinicians to enhance the patient experience through the delivery of excellent patient focussed care within the Virtual Ward. 

The post holder will be a highly competent, visible, and experienced autonomous practitioner who uses specialist knowledge and advanced skills to support the new way of working that emphasises a more efficient and patient focused service. 

The post holder will initially be based in one of Cornwall's CATUs to enable face to face review of patients on the virtual ward. They will also take part in daily ward rounds and review and amending treatment plans as required

Main duties of the job

The post holder will be able to take a relevant history; perform a holistic physical and mental health examination; undertake clinical assessment to establish a management plan and evaluate the progress of that plan within the limitations of their scope of practice.  This will be carried out under the non-direct supervision of a medical mentor. 

They will demonstrate advanced clinical judgement and critical decision-making skills based upon evidence-based practice.

The post holder will use specialist knowledge and advanced skills to support the new way of working that emphasises a more efficient and patient focused service.  This will ensure the safe referral or discharge of patients with undifferentiated and undiagnosed presentations in any area of the urgent treatment centre.

Person specification

Education / Qualifications and Relevant Experience

Essential criteria
  • Current professional registration as an RN or AHP with NMC or HCPC
  • Substantial experience at band 6 or 7 working with older people, assessing and managing individuals with long term conditions and frailty
  • Completion of/or currently undertaking the Degree level long term conditions module, or RCN accredited Nurse Practitioner diploma or the equivalent. Or evidence of undertaking advanced clinical examinations and making independent clinical decisions for complex and unpredictable presentations.
  • Post graduate teaching qualification or equivalent
Desirable criteria
  • Current Non-medical Prescriber or currently undertaking the module
