An exciting opportunity has arisen in the Tees North Intensive Home Treatment Team for a Band 3 Community Support Worker. 

The successful candidate will work closely with patients whom are presenting in a crisis in order to work towards recovery. They will  work as part of a wider MDT, and have the opportunity to liaise with both primary and secondary services. 

With management and clinical support from a dedicated leadership team, the successful candidate will receive regular supervision and appraisals, with a structured job plan and opportunities for progression. 

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010

Prif ddyletswyddau'r swydd

Day to day duties of the role will focus on patient review, completing physical health checks, documentation, care plans and safety plans, answering the phone and liaising with other services.

The successful candidate will be supported and overseen by the MDT and leadership team for a clear escalation route of any concerns and risks arising from the above duties. 

This post is not open to Trainees therefore the successful candidate must hold a Level 3 Diploma in Health and Social care or equivalent alongside Key Skills in Literacy and Numeracy Level 2.

Qualifications

  • Diploma in Health and Social Care level 3 or equivalent
  • Key skills in Literacy and Numeracy level 2
  • Successful completion of Trust’s full HCA Framework within 6-month probationary period

Experience

  • Previous experience of working with people with mental/social health needs or learning disabilities
Meini prawf dymunol
  • Experience of working in Physical Health, HDAT, Lithium and Clozapine Clinics
  • Experience in venepuncture
  • Experience of using a Clozapine POTCHI machine

Knowledge

  • Person centred care
  • Knowledge of Microsoft Office applications (within agreed timescale)
  • Knowledge of the CQUIN Physical Health targets (within agreed timescale)

Skills

  • Effective verbal and written communication skills
  • Competency in ECG and venepuncture (or to complete within agreed timescales)
  • Ability to take general observations i.e. pulse, BP, temperature, weight and height
  • Time management and ability to prioritise effectively
