A fantastic and exciting opportunity has arisen within Central and North West London NHS Foundation Trust for a highly motivated and enthusiastic Band 7 Team Leader to join our Community Mental Health Hub North at Mill House.

The post is a Band 7 Team Leader post – open to the professions of Mental Health Nursing, Occupational therapy and Social Workers.  The role consists of leadership/supervision of the CMHH staff group, duty management, a small specialist clinical caseload, MDT meeting facilitation and support/deputation to the CMHH Team Manager.

The post is for 37.5 hours a week and the CMHH working hours are Monday-Friday from 9am to 5pm.

  • To provide day-to-day management support of the multi-disciplinary team in delivering community care to service users and their carers based on an intervention-based model of care
  • To work closely with the Team Manager and other senior workers to develop the Community Mental Health Team
  • To establish strong working relationship with the in-patient Service, Hillingdon HTT, Hillingdon Psychiatric Liaison Team and CNWL Single Point of Access.
  • To attend any required meeting to update on CMHT actions and feedback to CMHT any further actions required.
  • To monitor workload and ensure fair distribution according to the various team roles. To ensure adequate cover of patient care during absences.
  • To have oversight of the health-related functions with the Community Mental Health Team
  • To chair the morning zoning meetings as required
  • To facilitate HTT transfers in a timely way
  • To have oversight of zoning for patients and their plans
  • To attend monthly interface meetings and contribute to improving local care pathways.
  • To regularly undertake delegated managerial responsibilities and duties, and to deputise for the Team Manager in his/her absence or as required
  • To undertake complaints and serious incident investigations in accordance with the Trust’s Policies.
  • To offer regular structured supervision to staff within the team, as well as providing informal support and advice to them and to any other team members in the course of their work.

CNWL have created multidisciplinary, health and social care Community Mental Health Hubs (CMHH) which are aligned to, and working closely with our GP Primary Care Networks (PCN) Colleagues. We work collaboratively with GP surgeries and acute Trusts aligned to the hub to provide a seamless service for patients of those surgeries and have close working relationships with local community organizations to help reduce health inequalities and improve health outcomes. 

The post holder will be a member of our Hillingdon Community Mental Health Hub delivering care to a set population locally. Our Community Mental Health Clinicians are responsible for assessing, monitoring and coordinating of our clients to ensure a high level of quality care is delivered within our available resources. We work in partnership, providing support and advice to users’ carers, family and other professionals regarding any mental health issues or concerns. 

The clinicians will support service users living in the community who have a range of mental health needs. Our Hubs are multidisciplinary teams working together to undertake or provide assessments, time-limited interventions, treatments and individually tailored care plans for all our clients. 

We work with adults of working age, who have acute or severe and enduring mental illness.

The CMHH is a multidisciplinary team, comprising of mental health nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists and peer support workers.  The team is supported by a good administrative system and offers an excellent work environment. The team has good links with local statutory and non-statutory services. The team works closely with our colleagues at The Riverside Centre at Hillingdon Hospital.

What we are looking for?

  1. Post registration experience with significant years of working with adults with mental health problems and experience of working with adults in a community setting.
  2. Leadership / supervisory experience.
  3. Excellent communication and interpersonal skills, with a sound understanding of current government legislation, demonstrate the ability to work as part of a MDT and provide high quality care with evidence based therapeutic assessments, interventions and completion of accurate and timely documentation.
  4. Specialist skills in mental health assessment, risk assessment, safety planning and medication administration.
  5. Car owner/driver would be ideal but not essential as it is about finding the right person

Job Purpose:

  • To undertake complaints and serious incident investigations in accordance with the Trust’s Policies.
  • To offer regular structured supervision to staff within the team, as well as providing informal support and advice to them and to any other team members in the course of their work.
  • To take an active role in keeping the CMHTS caseload fluid inclusive of Care Programming Approach, Lead professional Care
  • To undertake regular caseload scrutiny for categories such as discharge, step down or step up care.   
  • To participate with the Team Manager in staff appraisals.
  • To work in support of the Team Manager in ensuring that required documentation, activity reports and quality standards are maintained within the Teams.
  • To ensure that Trust policies are implemented, including clinical policies, Health and Safety, Fire, Sickness, the reporting of Untoward Incidents and Accidents, and to ensure that the Team Manager is informed of any such incident or any complaint. To assist in the investigation of such matters, at the Service Manager’s request.
  • To participate in the recruitment of junior staff.
  • To participate fully in team meetings.
  • To participate in the Team professional development activities.
  • To provide a learning environment for staff, students and visitors
  • To support the provision of high-quality placements for students from a variety of disciplines
  • To maintain clear, accurate and up-to-date recording of client work.
  • To work according to the principles of equal opportunities
  • To support the involvement of users and carers in discussions about their treatment and care as appropriate and help their voice to be heard.
  • To undertake regular audit of client case notes to ensure high standards of delivery of care are achieved and maintained

Job Flexibility

The postholder will be required to work flexibly, providing assistance as and when necessary, which may involve them in a developing role.
