Location/Department: home-based with frequent travel
Salary: £38,000 + £3,000 Car Allowance

About the opportunity
An exciting opportunity has arisen for enthusiastic, motivated, and innovative CHC Assessors to join our dedicated Clinical Team. If you are a looking for a highly flexible and autonomous position and are a qualified and registered professional as the following: Registered Nurse, Occupational Therapist, Physiotherapist, Social Worker, Disability Assessor/Analyst, CHC Nurse with experience in Continuing Healthcare then we have multiple positions to suit you and your lifestyle!
What you will be doing
- Delivering robust assessments & recommendations regarding eligibility for NHS CHC.
- Conducting Assessments and reviews in a number of settings incl. patients own homes, residential and nursing environments or within a hospital setting.
- Provide high quality support including complex information & analysis
- Provide a high level of internal & external communication
- Manage stakeholder relationships
We believe to be successful within this role, you will need to have
- Professional registration as RGN, RMN, RNLD, Physiotherapist, Occupational Therapist
- Experience in a Continuing Healthcare setting
- Experience working with NHS Commissioners, Local Authorities & Health/Social care providers.
- Assessment experience.
- Broad based clinical knowledge
- Safeguarding knowledge
- Proficient in MS Office programs
- Preferred CHC and in-house systems knowledge
