Are you a Band 5 Community Nurse who would like be part of new initiative in the Primary Care Home Team which covers Twinwoods Community Nursing Team? We are setting up a Triage Team and would like to incorporate a Rapid Response Nurse who will able to completed unplanned visits for the Community Nursing Team once they are triaged. This could include blocked catheters, palliative care interventions such as administration of stat doses for symptom control, new areas of pressure damage or urgent wound care needs.
We are looking for a dynamic nurse who is motivated to deliver high-quality care. Excellent communication and time management skills, and sound clinical knowledge, is essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development. You will be expected to work closely and deputize for the Triage Team Nurses as required as well as support the Community Nursing teams with routine visits depending on capacity.

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering Monday to Friday, 09:00 17:00. Post is permanent / fulltime.
