West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

This is an exciting opportunity for a Specialist IV Nurse nurse to join the Ealing Community Partnership. We are looking for a dynamic and motivated practitioner to join our team and lead on IV therapy in the community. Furthermore, we are also seeking a candidate who has experience, knowledge and proven skills in operational management to lead on the community phlebotomy service.

 

 

Community IV Specialist Nurse

 

This is a key role in the provision of an integrated specialist nursing service for residents requiring IV therapy who are registered with a GP in the London Borough of Ealing. This is a 7-day service from 9-5pm offering domiciliary visits.

 

The post holder will co-ordinate the delivery of IV therapy by the Adult Community Nursing Services and where possible it will be expected that the post holder will undertake the initial visits either alone or in conjunction with a member of one of the Community Nursing Teams.

 

Community Phlebotomy Service

 

The post holder will also co-ordinate the delivery of Community Phlebotomy supporting and leading in the development of the phlebotomy service in Ealing for housebound clients including those residing in care homes (residential). Facilitating the provision of high-quality care for clients within the community ensuring evidence-based practice and innovative models of care are always delivered. Evaluating clinical pathways, policies, and procedures for phlebotomy across primary and community care.

Intravenous Therapy 

·       Be the specialist lead for IV Therapy for the organisation, providing specialist informed advice to staff, clients and their carers/families carrying out where possible the initial visit either alone or in conjunction with a member of one of the Adult Community Nursing Teams. 

·       Work as an autonomous practitioner, using independent prescribing where appropriate managing and prioritising a complex caseload independently and within the multi-disciplinary team. 

·       Undertake the triage of referrals, initial assessment of patients and review assessments, to ensure that appropriate patients are accepted onto the caseload, and the most effective care package is designed and delivered that optimises patient care in the resources available. 

·      Develop, redesign, and assess care pathways, to provide timely intervention for clients requiring IV Therapy in the community. 

·       Monitor and lead on the formulation and review of policies, standards, and guidelines for IV therapy and act as an expert to implement and disseminate any changes made. 

 Phlebotomy

Be the specialist lead for Phlebotomy for the organisation, providing specialist informed advice to staff,

Develop, redesign, and assess care pathways for venepuncture.

Monitor and lead on the formulation and review of policies, standards, and guidelines for phlebotomy and act as an expert to implement and disseminate any changes made.

Qualifications

  • Qualified RGN (Registered General Nurse - Adult) with current registration with NMC - Part1.
  • Relevant post registration study at diploma/degree level and relevant CPD e.g., IV administration, venepuncture and cannulation, venous access device/long lines, use of elastomeric pump.
  • Experience working in the acute or in the community setting for more than a year.
  • Assessor/Supervisory/ Mentorship Preceptorship or equivalent (willing to undertake)

  • Diploma or BSc (Hons) or MSc in Community Health (DN)
  • Clinical Assessment Skills (Willing to undertake)
  • Nurse Prescriber V300 (willing to undertake)
