Senior Mental Health Professional

We have an opportunity for an experienced clinician to join our supportive and friendly team based in the Newbury locality.
We welcome applications from Registered Mental Health Nurses who have a passion to work with people of Older Age experiencing functional mental health difficulties and dementia (all ages) in the community setting.
You will be working in a Multidisciplinary Team who support each other to ensure that you are able to offer the best standard of care to our patient group.
You will primarily be working with the Home Treatment Team and will also work across all teams within the service as needed. You will take responsibility for the assessment and implementation of care and treatment in the homes of people experiencing functional mental health problems, as well as people with dementia. The successful candidate will be responsible for managing their own caseload, working collaboratively with other disciplines from both statutory and voluntary sectors as well as contributing towards the development of the service.
You will be based at Hillcroft House, located on the West Berkshire Community Hospital site, situated between Newbury and Thatcham. Our team operates Monday to Friday, which may involve varying shifts (9-5pm and 11-7pm Monday to Friday, with some weekend work (Saturday and Sunday 9-3).
This role requires multiple community patient visits throughout West Berkshire which is a vast geographic area.

To manage and be responsible for an identified caseload of patients.

To undertake initial, comprehensive assessments in a range of settings considering the person's physical, psychological, social and spiritual needs.

To develop an agreed plan of care with the service user in association with families and carers as appropriate.

To initiate and carry out the agreed treatment plan.

To monitor and evaluate the agreed care plan, adjusting as appropriate in collaboration with the wider team.

To develop and maintain effective communications with GP’s, Local Authority and Voluntary Organisations.

To participate actively in Service Development to enhance the nursing contribution and quality of care.

To maintain accurate clinical documentation as per guidance.

To ensure adherence to Trust policies.

To develop one area of specialist interest within the work of Department of Old Age Psychiatry.
