Community Mental Health Practitioner

The Intensive Home Treatment Team is a citywide service for older people experiencing an acute episode of a wide range of mental health difficulties and younger people with a diagnosis of Dementia who are in crisis. It is based at The Mount. IHTT is made up of Nurses, Occupational Therapists, Psychologists, Social Worker, Health Support Workers and Doctors. The team provide robust alternatives to admission; facilitate early transfer from hospital, as well as preventing the breakdown of a caring situation both at home or in a residential or nursing care setting when the Care Homes Service is unable to attend. Service operating hours, 8am-9pm, 7 days per week. Essential car driver and access to a vehicle.

You will work as part of a multi -disciplinary team providing assessment, care planning and care delivery, This will include the provision of highly skilled care, psychological interventions, clinical assessment, role specific assessments, developing plans of care and safety plans, risk assessments and carry out home based treatment interventions, assessments/risk management, and promoting recovery and inclusion. You will be involved in managing a team caseload and you will be required to work autonomously. IHTT operates over a 7 day week and you will be required to work extended hours (12 hours shifts) over this period including weekends and Bank Holidays.
