Job overview

The  Diabetes Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. 

The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice.  

The post holder will promote optimum independence and individualised client centred care to a delegated caseload.

The post holder will also support the Community Diabetes Team to deliver against key performance indicators and maintain quality of the service.

The post holder will be responsible for:

Main duties of the job

  • Previous experience working in a healthcare or community care setting,
  • Experience in conducting health checks, monitoring blood glucose, blood pressure, and weight
  • Familiarity with patient record systems 
  • Experience providing lifestyle advice on diet, exercise, and self-care 
  • Working effectively in a multidisciplinary team

Education/Qualification

Essential criteria
  • Higher Professional Diploma in Health and Wellbeing (NVQ/QCF – Level 3) or A recognised equivalent course
  • Willingness to undertake further training as appropriate
Desirable criteria
  • GCSE grade A-C or equivalent in English and Maths ECDL certificate
  • Registration with the national regulatory body (when it becomes available).

Experience

Essential criteria
  • Experience of working in a health or social care setting
  • Experience of working as part of a team
  • Experience with the use of Microsoft Office and a variety of databases.
Desirable criteria
  • Experience of working as part of a team
  • Experience with the NHS patient information database (Rio, CIS)

Skills & Knowledge

Essential criteria
  • Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy.
  • An awareness of risk management.
  • Knowledge of multi-agency services available and processes for referral
  • Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
Desirable criteria
  • Knowledge of local health needs strategies.
