As an integrated mental health, learning disability and community Trust, Lancashire and South Cumbria NHS Foundation Trust provides a range of services including:
• Primary and secondary mental health care for children and adults including specialist inpatient child and adolescent mental health provision, perinatal mental health, forensic services including low and medium secure care.
• Specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Community physical health and well-being services for children and adults.
The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites. We are seeking a Triage Clinical Care Co-ordinator to join our Care Connexions team , providing timely support to people experiencing health or social care crises in their own homes.
About the Role:
The post holder will support the triage processes and allocation and co-ordination of the clinical response for Care Connexions. You will provide clinical support to the care co-ordinators supporting them triage appropriately and safely and ensuring seamless co-ordination of services. You will support timely access to hospital avoidance services including 2 hr Urgent Community Response, Virtual Wards, Home First Service and 3rd sector
Working Hours:
37.5 Hours over a 7-day working week covering 8am-8pm

They will have experience working across Health and Social care settings and an understanding of hospital avoidance services to ensure the right services are deployed to best support the needs of the patient.

- • Evidence of continuing professional education or development in area of speciality
Knowledge

- • Specialised knowledge in discipline to ensure effective and accurate triage and assessment of patients
- • Significant experience in working in a MDT environment, liaising with multiple service providers ensuring appropriate care provision and planning.
- • Post degree learning and experience in relevant area
- • Evidence of leadership experience and learning.
- • Awareness of recent advances within NHS.
