Trosolwg o'r swydd
- To provide specialist assessment for patients referred by Leeds Teaching Hospital Trust who have coexisting physical, mental health and addiction problems.
- Provide a liaison role to LTHT, LYPFT and Forward Leeds for service users that sit within any one of these services to ensure that they receive the best evidence-based treatment.
- To contribute to assisting individuals in addressing their alcohol and drug use, through the implementation of evidence based psychosocial interventions and initiation of pharmacotherapies leading to referral to the wider Forward Leeds Service on discharge from hospital.
- Provide Specialist advice and support to Leeds Teaching Hospital Trust staff, who are looking after patients with coexisting physical, mental health and addiction problems, during their presentation to Leeds teaching Hospital Trust sites.
- Provide 7 day per week service (shifts include 1 weekend per month and occasional Bank Holiday cover)

Work as part of the existing Forward Leeds Hospital In-reach team within Leeds teaching Hospital to provide a 7 day week service (includes working weekends once per month and occasional Bank Holidays).
Screen referrals for patients with coexisting physical, mental health and substance use needs, received from Leeds Teaching Hospital Trust Staff.
Prioritise assessments based on needs, to support timely patient flow for Leeds Teaching Hospital Trust.
Co-ordination of shift.
Provide specialist assessment of patients referred to HIRT
Engage new patients to address substance use via Community Forward Leeds, using evidence based motivational dialogue.
Provide a liaison role for patients already in treatment with Community Forward Leeds service.
Provide support/Advice/education to Leeds Teaching Hospital staff regarding the best management for patients admitted to hospital who are experiencing substance use issues.
Contribute to Multidisciplinary decisions regarding the management of care for Patients in Hospital experiencing substance use issues
Advice on the initiation of opiate substitution therapy, alcohol withdrawal medications and relapse prevention medications within Leeds teaching Hospital Trust.
Contribute to service development.
Undertake the student assessor/Supervisor role where appropriate
Contribute to engagement of new patients wanting to access community service post-discharge.

Experience

- Two years post Qualification experience is desirable. Some experience of working with the client group is essential.

- Demonstrates competence and confidence, to provide expert advice, Knowledge of Addiction pharmacology and Psychosocial interventions essential.
